Alcohol

· Severity: Major

· Notes for Consumers: Do not drink alcohol while taking this medication. Drinking alcohol while taking this medication increases the risk for dangerously low blood pressure. Contact your care team right away if you experience dizziness, feeling faint or lightheaded, or blurry vision.

· Notes for Professionals: Inform patients that substantial consumption of alcohol (e.g., greater than 3 units) in combination with avanafil may increase the potential for orthostatic signs and symptoms, including increase in heart rate, decrease in standing blood pressure, dizziness, and headache. Both alcohol and PDE5 inhibitors including avanafil act as vasodilators. Concomitant use may attenuate the blood-pressure-lowering effects of each individual compound.

Grapefruit juice

· Severity: Moderate

· Notes for Consumers: Grapefruit juice and grapefruit-containing foods may increase the side effects of Avanafil. Avoid grapefruit juice if possible, or do not significantly alter grapefruit juice intake while taking this drug. When taking Avanafil, contact your prescriber immediately if you experience chest pain, dizziness, fainting or falling spells, palpitations, shortness of breath, or a change in your usual heart beat (fast, slow, or irregular heart beat).

· Notes for Professionals: Avanafil serum concentrations may increase with concurrent consumption of grapefruit juice and grapefruit-containing foods; it is possible that avanafil-induced side effects could also be increased in some individuals, although this specific interaction have not been studied. Avanafil is a primary substurate of CYP3A4. Studies have shown that drugs that inhibit CYP3A4 can increase avanafil exposure. Grapefruit juice has been reported to decrease the metabolism of drugs metabolized via this enzyme. Grapefruit juice contains a furano-coumarin compound, 6,7dihydroxybergamottin that inhibits CYP3A4 in enterocytes in the GI tract.
