Metyrapone is an oral diagnostic agent that inhibits steroidogenesis. Specifically, metyrapone works by inhibiting endogenous adrenal corticosteroid synthesis, thus decreasing the production of cortisol and corticosterone. Metyrapone is utilized to evaluate hypothalamic-pituitary ACTH function; the two regimens approved for use are a single-dose overnight regimen and a multiple dose regimen administered over 24 hours. In children with asthma receiving inhaled corticosteroids, it has been suggested that the overnight metyrapone test will detect abnormalities in adrenal function more often than other tests. Metyrapone is also used off-label in the treatment of Cushing's syndrome and is one of the preferred medications for use in patients with mild disease and no visible tumor on MRI. Guidelines also strongly recommend metyrapone in patients who require rapid normalization of cortisol as response is typically seen within hours. Treatment with metyrapone also results in general improvement of the clinical symptoms of Cushing's syndrome, such as blood pressure, glucose metabolism, and muscle weakness.

Oral Administration
-Administer with milk, yogurt, or a snack.
-Usually given on an outpatient basis; however, if adrenal insufficiency is suspected, hospitalization is recommended.
Metyrapone has been reported to cause gastrointestinal (GI) disturbances such as nausea, vomiting, and abdominal pain or discomfort. Nausea is a fairly common side effect reported by patients treated with the drug for Cushing's syndrome. It may also cause acute adrenal insufficiency in patients with reduced adrenal secretory capacity, and nausea, vomiting, and abdominal pain may occur in patients with adrenal insufficiency. Carefully evaluate any patient with GI symptoms to identify or rule out adrenal insufficiency.
Central nervous system effects reported with metyrapone use include headache, dizziness, and drowsiness or sedation. Dizziness is a commonly reported side effect by patients treated with the drug for Cushing's syndrome.
Hirsutism, acne vulgaris, edema, hypertension, fatigue, lethargy, arthralgia are other side effects that have been reported with use of metyrapone in the treatment of Cushing's syndrome during clinical studies. Reports of allergic rash or atopic dermatitis have also occurred. A decrease in dose from 3 grams/day to 2 grams/day corresponded with a reduction in the frequency of some side effects in some studies. Guidelines note that androgenic side effects can limit prolonged metyrapone treatment for Cushing's syndrome in female patients.
Anemia, leukopenia, and/or thrombocytopenia have been reported with metyrapone use.
Hypotension has been reported with metyrapone use. Carefully evaluate any patient with hypotension to identify or rule out adrenal insufficiency. In patients with reduced adrenal secretory capacity, metyrapone may cause acute adrenocortical insufficiency and hypotension. Other symptoms of acute adrenal insufficiency include cardiac arrhythmias, dehydration, nausea, vomiting, epigastric pain, diarrhea, anxiety, confusion, weakness, impaired consciousness, hyponatremia, hypochloremia, and hyperkalemia.
Metyrapone is contraindicated in patients with metyrapone hypersensitivity or hypersensitivity to any of its excipients.
Metyrapone is contraindicated in patients with adrenal insufficiency. Metyrapone may induce acute adrenal insufficiency in patients with reduced adrenal secretory capacity. Before administering metyrapone, assess the ability of the adrenals to respond to exogenous adrenocorticotropic hormone (ACTH).
In the presence of hypothyroidism or hyperthyroidism, response to metyrapone may be subnormal.
Because metyrapone may cause dizziness and sedation, advise patients to use caution when driving or operating machinery.
Discontinue all medications that affect pituitary or adrenocortical function, including all corticosteroid therapy, prior to and during testing with metyrapone.
Use metyrapone during pregnancy only if clearly needed. It is not known if metyrapone can affect reproduction capacity. A subnormal response to metyrapone may occur in pregnant women. When metyrapone was administered to 20 pregnant women in their second and third trimesters of pregnancy, the fetal pituitary gland responded to the enzymatic block. No medication for Cushing's syndrome is specifically approved for use during pregnancy. Treatment guidelines suggest, based on low quality evidence, that in pregnant women (or those desiring pregnancy), that cabergoline or metyrapone use may be considered.
Use caution when administering metyrapone to a breast-feeding woman. In a single case study, average breast milk concentrations of metyrapone and its metabolite at steady-state in a woman receiving 1,000 mg/day were 11 mcg/L and 48.5 mcg/L, respectively; a fully breast-fed infant would receive 0.1% of the weight-adjusted maternal dose, which would be unlikely to significantly affect the infant.
For hypothalamic-pituitary-adrenal (HPA) suppression diagnosis:

Adults: 500 mg to 6 grams/day PO, divided into 3 to 4 doses, is the recommended daily dosage range per treatment guidelines. In retrospective studies, urinary-free cortisol (UFC) normalization was approximately 70% with metyrapone. In a prospective study UFC normalization was 47% at week 12 with a median dosage of 1500 mg/day.

Mitotane: (Major) Mitotane suppresses the activity of the adrenal cortex, and may reduce the adrenal response to metyrapone. The manufacturer of metyrapone recommends that any drug that affects adrenal function be discontinued prior to and during metyrapone testing.
Molindone: (Moderate) Metyrapone may cause dizziness and/or drowsiness. Other drugs that may also cause drowsiness, such as molindone, should be used with caution. Additive drowsiness and/or dizziness is possible.

Under normal conditions, corticotrophin-releasing hormone (CRH) and arginine vasopressin (AVP) are released from the hypothalamus. CRH stimulates the pituitary to release adrenocorticotropic hormone (ACTH), which then stimulates the adrenal gland to increase production and release cortisol. Cortisol, via negative feedback, suppresses the release of ACTH.
Metyrapone inhibits the 11-beta-hydroxylase enzyme, which is responsible for the conversion of 11-desoxycortisol and 11-desoxycorticosterone to cortisol and corticosterone, respectively. Secondary to inhibition of 11-beta-hydroxylase, plasma levels of cortisol (a suppressor of ACTH release) decrease while levels of 11-desoxycortisol and 11-desoxycorticosterone increase. Both 11-desoxycortisol and 11-desoxycorticosterone weakly suppress the release of ACTH; thus, ACTH levels increase following the administration of metyrapone. There is also an increase in the urinary excretion of 17-hydroxycorticosteroids and 17-ketogenic steroids, metabolites of 11-desoxycortisol and 11-desoxycorticosterone. Following administration of metyrapone, plasma levels of 11-desoxycortisol and ACTH or urinary concentrations of 17-hydroxycorticosteroids are measured to evaluate the hypothalamic-pituitary ACTH function. Metyrapone is also believed to cause mild natriuresis through the inhibition of the synthesis of aldosterone.
Metyrapone is administered orally. It is metabolized to metyrapol, an active alcohol metabolite. Both metyrapone and metyrapol undergo conjugation with glucuronide. The elimination half-life of metyrapone is 1.9 +/- 0.7 hours while metyrapol takes twice as long to be eliminated. Within 72 hours after the administration of the first 4.5 g dose, administered as 750 mg every 4 hours, an average of 5.3% is excreted in the urine as metyrapone (9.2% as free drug and 90.8% as glucuronide) and 38.5% as metyrapol (8.1% as free metyrapol and 91.9% as glucuronide).

Metyrapone has a rapid onset with peak plasma concentrations reached 1 hour after oral administration. The peak plasma concentration is 3.7 mcg/ml and 7.3 mcg/ml following administration of 750 mg and 2000 mg of metyrapone, respectively. The plasma ratio of metyrapone to metyrapol is 1:15 at 8 hours after oral administration.
