Diazoxide is structurally similar to the thiazide diuretics. It is used parenterally as an antihypertensive and orally as an antihypoglycemic agent. As an antihypertensive, diazoxide is used for the short-term, acute reduction of blood pressure in patients with malignant hypertension. The oral formulation is indicated for the management of hypoglycemia secondary to hyperinsulinemia. Oral preparations of diazoxide are not suited for chronic management of hypertension because of the drug's hyperglycemic and sodium-retaining properties. Diazoxide was approved by the FDA in January 1973.

Intravenous Administration:

-No dilution necessary.
-Patient should be in the supine position prior to and immediately following IV injection. Keep patient in recumbent position for at least 60 minutes following injection. If administered concurrently with loop diuretics, patient should remain supine for 8-10 hours.
-Inject into a peripheral vein over 30 seconds or less. Take standing blood pressure prior to ambulation.
-Patients should be switched to oral administration as soon as possible.
Adverse reactions with diazoxide vary with the route of administration. In general, more serious reactions occur following IV administration than with oral administration, but this may reflect the different indications, dosage and condition of the patient. The incidence of adverse reactions were originally published when 300 mg IV bolus doses were routinely given. The most common adverse effects at this time were hypotension (7%), nausea and vomiting (4%), dizziness, and weakness (2%). Using the present, lower dosages, these effects are thought to be less common and less severe. Presently, hypotension is uncommon in patients being treated with intravenous diazoxide alone; but is common when diazoxide is used in combination with diuretics or other drugs with hypotensive effects. Severe hypotension may necessitate treatment with sympathomimetic agents. Hypotension occurs occasionally in those receiving oral diazoxide for hypoglycemia.
Sodium retention and fluid retention can occur frequently in patients receiving multiple doses of diazoxide, resulting in edema, weight gain and congestive heart failure (especially in uremic patients); diuretic therapy usually reverses the retention.
Hypertension both has been reported occasionally in those receiving oral diazoxide for hypoglycemia. Myocardial and cerebral ischemia can occur in patients receiving IV diazoxide. Myocardial ischemia may present as angina, ECG changes, atrial arrhythmias, or ventricular arrhythmias. Transient sinus tachycardia, sinus bradycardia, palpitations, severe hypotension, orthostatic hypotension, and chest discomfort or non-anginal chest pain (unspecified) have been observed following IV administration.
Pulmonary hypertension was reported in 11 neonates and infants treated with diazoxide for low blood sugar; in all cases, the pulmonary hypertension resolved or improved after diazoxide was stopped. Neonates and infants with the following risk factors may be at higher risk for the development of pulmonary hypertension: meconium aspiration syndrome, respiratory distress syndrome, transient tachypnea of the newborn, pneumonia, sepsis, congenital diaphragmatic hernia, and congenital heart disease. Instruct parents and caregivers to monitor their child carefully and seek immediate medical attention if the infant displays any signs of difficulty breathing.
Hyperglycemia is more likely to occur after IV administration of diazoxide, although it can also occur through overdosage via the oral route. Diabetic ketoacidosis and hyperosmolar coma may develop quickly. Restoration of fluid and electrolytes, together with administration of insulin is usually effective. Because diazoxide has a long half-life, patients should be monitored for a prolonged period until blood glucose stabilizes. A reduction in dosage at the first appearance of hyperglycemia may avoid progression to ketoacidosis and coma.
Hirsutism of the lanugo type has been reported with oral and intravenous use of diazoxide. Hypertrichosis occurs mainly on the forehead, back and limbs, and is more common in children and women. Withdrawal of diazoxide usually results in a regression.
Adverse gastrointestinal effects with diazoxide include anorexia, nausea, vomiting, dysgeusia (or a transient loss of taste), abdominal pain, diarrhea, constipation, xerostomia, salivation, parotid swelling, and ileus. Acute pancreatitis has occurred in one case in a patient with renal failure on long-term therapy.
Sensitivity reactions, such as skin rash (unspecified) and leukopenia fever have occurred after IV administration of diazoxide. Fever has been reported after oral and IV administration of diazoxide. The exact incidence of these effects is not known.
Thrombocytopenia due to diazoxide is more likely to occur after prolonged oral administration, and may be accompanied by purpura. Thrombocytopenia may require discontinuation of therapy. Neutropenia due to diazoxide is usually of a transient nature and not accompanied by an increased susceptibility to infection.
Extravasation of diazoxide during IV administration can cause burning, cellulitis, or phlebitis. A feeling of warmth or pain can occur along the injected vein. Patients receiving IV diazoxide should be monitored for an injection site reaction.
Adverse central nervous system (CNS) effects and dystonic reaction have been reported during prolonged oral administration of diazoxide and include anxiety, insomnia, polyneuritis, and paresthesias. Cerebral ischemia has been reported with the intravenous use of diazoxide and is usually transient but can occasionally cause infarction manifested by unconsciousness, seizures, muscle paralysis, confusion, or focal neurologic deficit such as numbness of the hands (paresthesias). Other CNS effects reported with intravenous diazoxide include those associated with findings secondary to alteration in regional blood flow to the brain such as headache (sometime throbbing), dizziness, lightheadedness, lethargy, drowsiness, euphoria, tinnitus, momentary hearing loss, weakness of short duration, apprehension, restlessness, tremor, and anxiety. Other adverse reactions reported due to the vasodilating actions of intravenous diazoxide include headache, flushing, pruritus, and diaphoresis.
Adverse events have been reported with intravenous and oral use of diazoxide and include transient cataracts, ocular hemorrhage, ring scotomata, blurred vision, lacrimation, diplopia, and optic nerve infarction following a too rapid decrease of severely elevated blood pressure.
Although not specifically reported with the intravenous use of diazoxide, hematologic reactions including eosinophilia, decreased IgG, decreased hemoglobin and hematocrit (anemia), and excessive bleeding have been reported with oral use. Lymphadenopathy has also been reported.
Elevated hepatic enzymes (AST, alkaline phosphatase) have been reported with the oral use of diazoxide.
Renal adverse reactions including decreased creatinine clearance and urinary output, azotemia, hematuria, albuminuria, and reversible nephrotic syndrome have been reported with the oral use of diazoxide. Hyperuricemia is relatively common; gout has also been reported.
Dyspnea, cough, and choking sensation, secondary to relaxation of smooth muscle, have been reported with the intravenous use of diazoxide.
Skeletal changes, specifically advance in bone age, has been reported with oral diazoxide. Alopecia, galactorrhea, and monilial dermatitis have also occurred.
Diazoxide can cause severe hyperglycemia. Patients receiving the drug should be monitored carefully for this effect, especially patients with diabetes mellitus or hepatic disease, or those receiving antihypoglycemic agents. It is recommended that blood glucose be monitored closely in patients receiving intravenous diazoxide. It is contraindicated as a treatment for functional hypoglycemia (e.g., reactive or post-prandial hypoglycemia; or hypoglycemia due to diabetic therapy). These conditions are more effectively treated with conventional measures. Hyperglycemic effects of diazoxide can be potentiated in patients with hypokalemia.
Diazoxide should be administered cautiously to patients in whom sodium and water retention could be detrimental (e.g., those with poor cardiac reserves, including those with uncompensated congestive heart failure). Sudden reductions in blood pressure secondary to diazoxide can be hazardous to patients with cardiac disease or coronary artery disease, renal disease, cerebrovascular disease or insufficiency, or mild tachycardia. Prolonged hypoglycemia can exacerbate renal failure.
Diazoxide should be used cautiously in patients with uremia because protein binding of the drug is reduced, and clinical effects can be more pronounced.
Use diazoxide with caution in neonates and infants, especially those with risk factors for pulmonary hypertension such as meconium aspiration syndrome, respiratory distress syndrome, transient tachypnea of the newborn, pneumonia, sepsis, congenital diaphragmatic hernia, and congenital heart disease. Pulmonary hypertension was reported in 11 neonates and infants treated with diazoxide for low blood sugar; in all cases, the pulmonary hypertension resolved or improved after diazoxide was stopped. Patients receiving diazoxide should be monitored for signs of difficulty breathing such as flaring nostrils, grunting, unusual movement of their child's chest, rapid breathing, difficulty feeding, or a bluish color of the lips or skin. Diazoxide should be stopped immediately if pulmonary hypertension is identified.
Diazoxide is contraindicated in the management of hypertension due to aortic coarctation or arteriovenous shunt. The drug is ineffective against these conditions, and diazoxide-induced increases in cardiac output can be harmful.
Diazoxide is contraindicated in patients with hypertension secondary to pheochromocytoma because the drug is ineffective against this disorder.
The half-life of diazoxide may be prolonged in patients with renal impairment. Accumulation of the drug and possible toxicity may result. Dosage adjustments may be required.
Diazoxide injection is contraindicated in patients with sulfonamide hypersensitivity; both the injection and oral suspension are contraindicated in patients with thiazide diuretic hypersensitivity. Chemical structure similarities makes cross sensitivity reactions possible.
Diazoxide is classified as pregnancy risk category C. Although no adequate human studies have examined the effects of this drug on the fetus, animal reproduction studies have shown adverse fetal effects. Therefore, in making the decision to administer this drug during pregnancy, the potential risks to the fetus must be weighed against the potential benefits to the mother. Intravenous diazoxide should not be used during labor and obstetric delivery because it can inhibit uterine contractions.
It is not known if diazoxide is found in breast milk. Because of the potential risk for adverse reactions in nursing infants, diazoxide is not recommended concurrently with breast-feeding.
Use diazoxide with caution in patients with a history of gout or hyperuricemia; diazoxide may cause an increase in serum uric acid concentrations.
Do not give diazoxide via subcutaneous administration or intramuscular administration. Administer IV diazoxide intravenously through a peripheral vein, taking care to avoid extravasation. Diazoxide injection is an alkaline solution and may be irritating to tissues.
For the treatment of hypertensive emergency:
NOTE: Diazoxide is not effective in treating hypertension resulting from pheochromocytoma or monoamine oxidase inhibitor therapy. Because diazoxide increases cardiac output, it may be harmful if used in patients with hypertension secondary to aortic coarctation or arteriovenous shunt.
Intravenous bolus dosage:
Adults: 1-3 mg/kg IV push (the maximum single dose is 150 mg) every 5-15 minutes as needed until desired blood pressure reduction is attained. Patient should be switched to oral therapy as soon as adequate control of hypertension is achieved. The use of a 300 mg IV bolus dose is no longer recommended because the response to this dosage is unpredictable.
Intravenous infusion dosage*:
Adults: A slow IV infusion of 15 mg/minute over 20 to 30 minutes (total dose: 5 mg/kg, minimal dose of 300 mg) has been studied in 81 hypertensive patients. Blood pressure was lowered effectively, but gradually. The decrease in MAP was only 5.7% at 5 minutes and reached a maximum of 25.3% at roughly 26 minutes. Diazoxide was effective regardless of degree of renal dysfunction. The decrease in MAP was significantly greater in patients with preeclampsia. The authors concluded that slow infusion is preferred to avoid the acute drop in BP that can occur with bolus IV administration.
For the treatment of secondary hypoglycemia or hypoglycemia due to congenital hyperinsulinemia in neonates and infants*:
NOTE: FDA-approved labeling states diazoxide should be used only after a diagnosis of hypoglycemia due to one of the following conditions has been definitely established. For adults: inoperable islet cell adenoma or carcinoma, or extrapancreatic malignancy. For children and infants: leucine sensitivity, islet cell hyperplasia, nesidioblastosis, extrapancreatic malignancy, islet cell adenoma, or adenomatosis. Diazoxide may be used preoperatively temporarily, and postoperatively, if hypoglycemia persists.
Oral dosage:
Adults: Approximately 3 mg/kg/day PO given in divided doses every 8 hours initially. The usual dosage range is 3 to 8 mg/kg/day PO given in divided doses every 8 to 12 hours. Higher doses may be necessary for refractory hypoglycemia. If diazoxide therapy is not effective in controlling hypoglycemia after 2 to 3 weeks, discontinue therapy.
Children and Adolescents: 3 mg/kg/day PO given in divided doses every 8 hours initially. The usual dosage range is 3 to 8 mg/kg/day PO given in divided doses every 8 to 12 hours. Higher doses may be necessary for refractory hypoglycemia. If diazoxide therapy is not effective in controlling hypoglycemia after 2 to 3 weeks, discontinue therapy.
Neonates and Infants: 10 mg/kg/day PO given in divided doses every 8 hours initially. The usual range is 8 to 15 mg/kg/day; however, 5 to 20 mg/kg/day PO divided every 8 to 12 hours has been reported. For congenital hyperinsulinemia, diazoxide is often given in combination with chlorothiazide (7 to 10 mg/kg/day PO divided every 12 hours) for its additive hyperglycemic effects and to minimize fluid retention associated with diazoxide. Higher doses may be necessary for refractory hypoglycemia. If diazoxide therapy is not effective in controlling hypoglycemia after 2 to 3 weeks, discontinue therapy. Pulmonary hypertension has been reported in neonates and infants receiving diazoxide; monitor patients for signs and symptoms of respiratory distress.

Alemtuzumab: (Moderate) Alemtuzumab may cause hypotension. Careful monitoring of blood pressure and hypotensive symptoms is recommended especially in patients with ischemic heart disease and in patients on antihypertensive agents.
Aliskiren: (Moderate) Aliskiren can enhance the effects of vasodilators on blood pressure if given concomitantly. This additive effect may be desirable, but dosages must be adjusted accordingly. Blood pressure and electrolytes should be routinely monitored in patients receiving aliskiren.
Aliskiren; Hydrochlorothiazide, HCTZ: (Moderate) Aliskiren can enhance the effects of vasodilators on blood pressure if given concomitantly. This additive effect may be desirable, but dosages must be adjusted accordingly. Blood pressure and electrolytes should be routinely monitored in patients receiving aliskiren. (Moderate) Enhanced hyperglycemia is possible during concurrent use of diazoxide and thiazide diuretics. Additive hypotensive effects can also occur with the concomitant administration of diazoxide with thiazide diuretics.
Allopurinol: (Moderate) Diazoxide can cause hyperuricemia. Dosages of concomitantly administered antigout medications, including allopurinol, may require adjustment.
Alogliptin: (Minor) Diazoxide, when administered intravenously or orally, produces a prompt dose-related increase in blood glucose level, due primarily to an inhibition of insulin release from the pancreas, and also to an extrapancreatic effect. Alprostadil: (Minor) The concomitant use of systemic alprostadil injection and antihypertensive agents, such as the vasodilators, may cause additive hypotension. Caution is advised with this combination. Systemic drug interactions with the urethral suppository (MUSE) or alprostadil intracavernous injection are unlikely in most patients because low or undetectable amounts of the drug are found in the peripheral venous circulation following administration. In those men with significant corpora cavernosa venous leakage, hypotension might be more likely. Use caution with in-clinic dosing for erectile dysfunction (ED) and monitor for the effects on blood pressure. In addition, the presence of medications in the circulation that attenuate erectile function may influence the response to alprostadil. However, in clinical trials with alprostadil intracavernous injection, anti-hypertensive agents had no apparent effect on the safety and efficacy of alprostadil.
Ambrisentan: (Major) Additive hypotensive effects can occur with the concomitant administration of diazoxide and ambrisentan. This interaction can be therapeutically advantageous, but dosages must be adjusted accordingly. The manufacturer advises that IV diazoxide should not be administered to patients within 6 hours of receiving other antihypertensive agents.
Amifostine: (Major) Patients receiving antihypertensive agents should be closely monitored during amifostine infusions due to additive effects. If possible, patients should not take their antihypertensive medication 24 hours before receiving amifostine. Patients who can not stop their antihypertensive agents should not receive amifostine or be closely monitored during the infusion and, possibly, given lower doses.
Amiloride: (Moderate) Additive hypotensive effects can occur with the concomitant administration of diazoxide with other antihypertensive agents. This interaction can be therapeutically advantageous, but dosages must be adjusted accordingly. If a medication-free interval is not feasible, initiate therapy at the lowest appropriate dose and monitor blood pressure closely.

Vincristine Liposomal: (Moderate) Use sodium phosphates cautiously with diazoxide, as concurrent use can cause hypernatremia.
Vitamin B Complex Supplements: (Moderate) Cutaneous vasodilation induced by niacin may become problematic if high-dose niacin is used concomitantly with other antihypertensive agents, especially peripheral vasodilators. This effect is of particular concern in the setting of acute myocardial infarction, unstable angina, or other acute hemodynamic compromise. The interaction is harmless unless niacin augments the hypotensive actions of clonidine.
Warfarin: (Minor) Diazoxide can displace highly protein-bound drugs from their protein-binding sites, resulting in an increased therapeutic effect. This interaction should be considered when administering diazoxide concomitantly with other highly protein-bound drugs such as warfarin.
Ziprasidone: (Minor) Ziprasidone is a moderate antagonist of alpha-1 receptors and may cause orthostatic hypotension with or without tachycardia, dizziness, or syncope. Additive hypotensive effects are possible if ziprasidone is used concurrently with antihypertensive agents.
Diazoxide causes vasodilation of peripheral arterioles, resulting in a reduction in peripheral vascular resistance and blood pressure. Diazoxide exerts its effects by hyperpolarizing (and effectively relaxing) arterial smooth muscle cells by activating ATP-sensitive potassium channels. The degree of vasoconstriction appears to be an important factor in the hypotensive actions of diazoxide. Patients with malignant hypertension appear to receive the most benefit. Consequently, as an antihypertensive, diazoxide is most effective in the acute setting. During long-term therapy, sodium and fluid retention offset diazoxide's vasodilatory effect. Reflex tachycardia occurs, although diazoxide has no apparent direct chronotropic or inotropic activity. Cerebral blood flow, coronary blood flow, and myocardial oxygen consumption remain unchanged.
Sodium retention, and subsequently fluid retention, are secondary to increased proximal tubular resorption of sodium and a reduced glomerular filtration rate. Renin increases during diazoxide therapy. Potassium, chloride, and bicarbonate excretion is reduced during diazoxide therapy, while uric acid excretion is enhanced.
Blood glucose increases during diazoxide therapy. Diazoxide inhibits pancreatic secretion of insulin, stimulates the release of glucose from the liver, and causes catecholamine release. Inhibitory effects on insulin release may be due to a change in calcium-ion flux across the cell membrane. Plasma free fatty acid concentrations are increased during diazoxide therapy. Insulin or tolbutamide will reverse the hyperglycemic effects of diazoxide, but diazoxide does not inhibit tolbutamide-induced insulin release.
Diazoxide is administered orally for the treatment of hypoglycemia and intravenously for the treatment of hypertension.
Diazoxide is 90% protein bound, and only unbound diazoxide is active. Diazoxide distributes throughout all body tissues, concentrating in the kidneys, liver, and adrenal glands, and it appears to cross the blood-brain barrier and the placenta. It is not known whether diazoxide is found in breast milk. Diazoxide is partially metabolized in the liver, and both unchanged drug and metabolites are excreted slowly in the urine. The extended half-life appears to be due to the extensive protein binding.

Due to the high protein binding (90%) of diazoxide, it was originally thought that diazoxide must be given by rapid IV injection to produce high serum drug concentrations in the plasma and at arterial receptor sites, before extensive binding occurs. Since then, reports have shown that slow (i.e., 15 mg/minute) intravenous infusions are also effective and reduce the risk of unnecessary hypotension. Smaller, 'mini bolus' doses are also routinely given which are safe and effective.
The elimination half-life is 21 to 36 hours following a single IV dose in patients with normal renal function, and it increases in patients with renal disease in proportion to the reductions in creatinine clearance (in patients with renal disease, diazoxide half-life ranges from 20 to 53 hours). The antihypertensive effects of diazoxide last from 4 to 20 hours following IV administration.

Limited data indicate that the half-life of diazoxide may be shorter in pediatric patients versus adult patients. After oral administration, the plasma half-life in 4 infants and children (4 months to 6 years of age) was reported to range from 9.5 to 24 hours after long-term administration compared to 24 to 36 hours in 2 adult patients.
