Benzoyl peroxide is a topical oxidizing agent with bactericidal, keratolytic, and sebostatic effects. It is FDA approved as a 5% topical cream (Epsolay) to treat acne rosacea. It is also available in multiple over-the-counter topical formulations for the treatment of acne vulgaris. Benzoyl peroxide is effective as monotherapy in mild cases of acne and is used as an adjuvant in moderate to severe cases of acne. The drug improves both inflammatory and non-inflammatory acne lesions. Benzoyl peroxide has also been used in the off-label treatment of decubitus ulcers.

Topical Administration

Cream/Ointment/Lotion Formulations
-Epsolay 5% cream:
--Before initial use, prime the pump until the first drop of cream is release.
-Clean and dry affected area before each application.
-Apply a pea-sized amount in a thin film to each area of the face (forehead, chin, nose, each check).
-Cream or lotion for acne:
--Clean the affected area thoroughly before applying.
-Apply enough medication to cover the area and rub in gently.
Other Topical Formulations
-BenzePro Foam:
--Prime container prior to first use by shaking the can vigorously. Hold the can upright over a sink. Direct initial spray to a non-skin surface. Press down on the actuator.
-Shake can vigorously before each use.
-Hold the can upright, dispense the foam into the palm of the hand and apply to the affected area.
-Wipe off any excess fam from the actuator after use.
-Wash hands with soap and water.
-BenzEFoam foam:
--Prime container prior to first use by shaking the can well. Firmly strike bottom of the can onto the palm of the other hand 3 times. Hold the can upright and press down on the actuator for 1 to 3 seconds until foam begins to dispense. Direct initial spray to a non-skin surface; DO NOT spray directly on the skin as the initial spray may expel cold liquid propellant.
-Shake can well before each use.
-Hold the can upright, dispense foam into palm of the hand or onto an applicator pad.
-Rinse applicator with water and allow it to dry after use.
-Wash hands with soap and water.
-Cleansing bar:-Using warm water, wash affected area for 1 to 2 minutes.
-Rinse thoroughly and pat dry with a clean towel.
-Emulsion:
--Clean the skin thoroughly before applying.
-Cover the entire affected area with a thin layer and rinse thoroughly.
-Foaming Cloths:-Wet face and cloth with water.
-Work cloth into a full lather.
-Cleanse face with the cloth for 10 to 20 seconds while avoiding the eyes.

-Throw away cloth; do not flush.
-Gel:
--Clean the skin thoroughly before applying.
-Cover the entire affected area with a thin layer and rinse thoroughly.
-Wash lotion:
--Wet skin and liberally apply to areas to be cleansed.
-Massage gently into skin for 10 to 20 seconds, working into a full lather.
-Rinse thoroughly and pat dry.
-If excess drying occurs, control by rinsing off cleanser sooner or using less often.
Local cutaneous adverse reactions (i.e., erythema, burning, itching, dryness, peeling, edema, allergic contact dermatitis) have been reported with the use of benzoyl peroxide products. During 2 clinical trials in adults receiving benzoyl peroxide 5% cream for acne rosacea, the majority of treatment-related adverse effects were mild to moderate in severity. During clinical trials, skin irritation (i.e., stinging/burning) was reported with a severity of mild, moderate, or severe in 20%, 3%, and 1% of patients, respectively. Skin dryness (xerosis) was reported with a severity of mild or moderate in 25% and 7%, respectively. Mild and moderate pruritus was reported in 24% and 6% of patients, respectively, and scaling of the skin was reported with a severity of mild or moderate in 13% and 4%, respectively. Additional application site adverse effects reported in clinical trials include pain (2%), erythema (2%), and eczema (1%). If irritation and contact dermatitis occur, consider treatment with moisturizers. If symptoms do not improve, discontinue benzoyl peroxide.
Topical over-the-counter (OTC) acne products, including benzoyl peroxide, have been associated with rare but serious and potentially life-threatening hypersensitivity reactions. These reactions may occur within minutes to a day or longer after use of the product. Instruct patients to stop using topical acne products if they experience signs of anaphylactoid reactions such as throat tightness; difficulty breathing; feeling faint; or swelling of the eyes, face, lips, or tongue. The product should also be discontinued in patients who develop urticaria or pruritus. Based on the information reported to the FDA, it is uncertain whether the reactions are caused by the active ingredients benzoyl peroxide or salicylic acid, the inactive ingredients, or a combination of both. When initiating therapy with an OTC topical acne product, advise patients to apply a small amount to one or two small affected areas for 3 days and monitor for signs of a hypersensitivity reaction. If no discomfort occurs, the instructions on the Drug Facts label may be followed.
Benzoyl peroxide is contraindicated in individuals having known sensitivity to benzoyl peroxide or any other listed ingredients. Hypersensitivity reactions, including anaphylaxis, angioedema, and urticaria, have been reported with the use of benzoyl peroxide. If a serious hypersensitivity reaction occurs, discontinue use immediately and initiate appropriate therapy.
Avoid sunlight (UV) exposure, including sunlamps, during treatment with benzoyl peroxide. Patients who may have considerable sun exposure due to their occupation and those patients with inherent sensitivity to sunlight should exercise particular caution when using benzoyl peroxide. If sun exposure cannot be avoided, broad spectrum sunscreen products and physical sun blocks (protective clothing, hats) are recommended for protection of treated areas. Discontinue use of the product at the first evidence of a sunburn.
Apply benzoyl peroxide only to affected areas; accidental exposure to unaffected skin may cause irritation. Instruct patients not to use more than the recommended amount and not to apply more often than as directed as this also may increase irritation. Avoid contact with the eyes, eyelids, lips, and other mucous membranes. If unintended mucus membrane or ocular exposure occurs, thoroughly rinse affected areas with water. Do not apply to skin that is cut or is affected with seborrheic dermatitis, eczema, a skin abrasion, or sunburn. Avoid concurrent use of other potentially irritating topical products. Not recommended for use in patients with preexisting skin sensitivity.
The safety and efficacy of benzoyl products in children have not been established.
Available data regarding the use of benzoyl peroxide during pregnancy are insufficient to establish a drug-associated risk of major birth defects, miscarriages, or other adverse maternal or fetal outcomes. Animal reproductive studies have not been conducted. Systemic exposure is unknown, and benzoyl peroxide is rapidly metabolized to benzoic acid (an endogenous substance) which is eliminated in the urine; therefore, maternal use is not expected to result in fetal drug exposure.
There are no data on the presence of benzoyl peroxide in human milk, the effects on a breast-fed infant, or the effects on milk production. Systemic exposure is unknown and benzoyl peroxide is rapidly metabolized to benzoic acid (an endogenous substance) which is eliminated in the urine; therefore, any amount of benzoyl peroxide excreted into human milk by a nursing mother would be rapidly metabolized by tissue and stomach esterases. To minimize potential exposure to a breast-fed infant, use benzoyl peroxide on the smallest area of skin and for the shortest duration possible while breast-feeding. Instruct patients not to apply to the area near the nipples, and be careful to avoid direct contact between the infant and the treated area. Only water-miscible cream products should be applied to the breast because ointments may expose the infant to high levels of mineral paraffins. Consider the benefits of breast-feeding, the risk of potential infant drug exposure, and the risk of an untreated or inadequately treated condition.

For the treatment and prevention of mild or moderate acne vulgaris:

Adults: Apply a thin layer topically to the affected skin area(s) 1 to 3 times daily. To minimize potential dryness, start with 1 application daily and gradually increase to 2 to 3 times daily. Reduce application to once daily or every other day if bothersome dryness or peeling occurs.
Adolescents: Apply a thin layer topically to the affected skin area(s) 1 to 3 times daily. To minimize potential dryness, start with 1 application daily and gradually increase to 2 to 3 times daily. Reduce application to once daily or every other day if bothersome dryness or peeling occurs.

Topical dosage (lotion):
Adults: Dosage and efficacy not firmly established. Small sample size and other methodological problems exist in the published literature. Most studies available do not support the routine use of systemic antibiotics for leg ulcers or diabetic foot ulcers without acute infection, and it is not clear if benzoyl peroxide or other peroxide agents provide improvements in efficacy versus saline alone; many results are equivocal. Ulcers studied include decubitus ulcers, leg or varicose ulcers, and diabetic foot ulcers. A 10% or 20 % benzoyl peroxide lotion, applied as part of wound management, followed by sterile dressing, has been used. Apply a sterile dressing or sponge moistened with normal saline and saturated with the 10% or 20% benzoyl peroxide lotion topically to the clean and debrided ulcer. Change the dressing as per standard of care. A protective ointment should be applied to a wide area bordering the ulcer to protect surrounding skin to help lessen contact irritation or dermatitis, which can be a common side effect.
For the treatment of inflammatory lesions of acne rosacea:
