Cholera vaccine is a live, weakened oral vaccine for the prevention of cholera caused by serogroup O1 in adults and pediatric patients 2 years and older traveling to cholera-affected areas. A single dose is administered at least 10 days before travel. Cholera, a disease caused by Vibrio cholerae bacteria, is acquired by ingesting contaminated water or food and causes watery diarrhea that ranges from mild to extremely severe. Although the infection is often mild, severe cholera, characterized by profuse diarrhea and vomiting, can lead to dehydration. Without adequate fluid replacement and antibiotics, severe cholera can be life threatening. Cholera is rare in the United States; however, travelers to parts of the world with inadequate water and sewage treatment and poor sanitation are at risk for infection. Vaccine efficacy in adults for preventing moderate to severe diarrhea is approximately 90% and 80% for patients at 10 days and 3 months post-vaccination, respectively. The efficacy of cholera vaccine for pediatric patients 2 years and older was demonstrated after comparison of the immune response to V. cholerae in children and adolescents to the immune response in adults after cholera vaccine (immunobridging). Cholera vaccine is the only vaccine available for the prevention of cholera.

-CAUTION: Observe and exercise cautions for handling, preparing, administering, and disposing of the cholera vaccine. Inactivate any spilled vaccine and clean any non-disposable equipment used in the preparation of the cholera vaccine with 70% isopropyl alcohol or 10% bleach solution.
Route-Specific Administration
Oral Administration
-Do NOT administer parenterally.
-Do not mix with any other vaccines or solutions.
-The patient may not have food or drink for 60 minutes before or after vaccine administration.
-Prepare and administer the cholera vaccine in a health care setting equipped to dispose of medical waste.
Oral Liquid Formulations
-If the packets are reconstituted in the improper order, the vaccine must be discarded.
-Locate the 2 packets; the buffer component (Packet 1) and the active component (Packet 2).
-Poor 100 mL of cold or room temperature (41 to 72 degrees F; 5 to 22 degrees C) bottled water (purified, spring, or sparkling [carbonated]) into a clean, disposable cup. Do NOT use tap/faucet water or any non-purified bottled water, other beverages, or other liquids.
-Using scissors, cut off the top of the buffer component packet and empty contents into cup. Effervescence will occur.
-Stir the buffer components with a disposable stirrer until completely dissolved.
--Children 2 to 5 years: Discard half of the mixed buffer solution (total dose = 50 mL).
-Using scissors, cut off the top of the active component packet and empty the active component packet contents (lyophilized V. cholera CVD 103-HgR) into the cup containing the buffer solution.
-Stir for at least 30 seconds and until active component disperses to form a slightly cloudy suspension that may contain some white particulates. The active component may dissolve completely.
-If desired, add no more than 4 grams (1 teaspoon) of sucrose (table sugar) or no more than 1 gram (1/4 teaspoon) of non-flavored stevia sweeteners. Once added, stir into the suspension. Do NOT add any other sweeteners or medicinal flavorings as this may reduce vaccine effectiveness.
-Consume the cholera vaccine within 30 minutes, if sucrose or non-flavored stevia are added. Consume the cholera vaccine within 4 hours of reconstitution, if sucrose and non-flavored stevia have not been added, at room temperature around 70 degrees F (21 degrees C).
-The recipient should drink the full contents of the cup at once. Some residue may remain in the cup and should be discarded with the cup.
The US Department of Health and Human Services has established a Vaccine Adverse Event Reporting System (VAERS) to accept all reports of suspected adverse events after the administration of any vaccine, including the cholera vaccine. The toll-free number for VAERS is 1-800-822-7967.
Adverse reactions associated with the cholera vaccine in adult clinical trials (n = 2,789) included fatigue (31.3% for cholera vaccine compared to 27.4% of placebo), headache (28.9% for cholera vaccine compared to 23.6% for placebo), and fever (0.6% for cholera vaccine compared to 1.2% for placebo). The incidence of severe adverse reactions in the patients receiving the cholera vaccine was 0.7% for fatigue, 0.5% for headache, and 0.11% for fever. Adverse reactions associated with the cholera vaccine in pediatric clinical trials (n = 468) included fatigue (31% to 41%), headache (9% to 45%), and fever (1.2% to 3.2%). The incidence of severe adverse reactions in the patients receiving the cholera vaccine was 0.6% for fatigue, 0.6% to 1.3% for headache, and 0.6% to 2.5% for fever.
Gastrointestinal adverse reactions were one of the most commonly reported side effects to the cholera vaccine in adults and pediatric patients. In adult clinical trials (n = 2,789), the most frequently reported GI adverse events included abdominal pain (18.7% for cholera vaccine compared to 16.9% for placebo), nausea and/or vomiting (18.3% for cholera vaccine compared to 15.2% for placebo), lack of appetite (anorexia) (16.5% for cholera vaccine compared to 16.6% for placebo), and diarrhea (3.9% for cholera vaccine compared to 1.2% for placebo). The incidence of severe adverse effects was 0.4% for abdominal pain, 0.3% for nausea and/or vomiting, 0.3% for anorexia, and 0.84% for diarrhea. In pediatric clinical trials (n = 468), the most frequently reported GI adverse events included abdominal pain (17% to 38%), lack of appetite (15% to 29%), nausea (7% to 22%), vomiting (1.4% to 5.5%), and diarrhea (0.7% to 3.6%). The incidence of severe adverse effects was 0.6% for abdominal pain, nausea, vomiting, and anorexia, and 1.8% for diarrhea.
Cholera may be shed in the feces of vaccinees for at least 7 days after administration; the duration of shedding of the vaccine strain is unknown. Instruct vaccine recipients to exercise caution during the 7-day period of viral shedding, especially when in close contact with non-vaccine recipients. Use caution when deciding whether to administer the vaccine to persons with immunosuppressed contacts. In addition, safety and efficacy have not been established in persons with immunosuppression; the immunologic response to the cholera vaccine may be diminished in these patients. Potentially immunocompromised patients include those receiving chemotherapy (antimetabolites, alkylating agents, or cytotoxic drugs) or radiation therapy and those on immunosuppressive therapy such as high-dose systemic corticosteroids. Corticosteroid therapy is usually not a contraindication to administering live-virus vaccines when administration is short-term (less than 2 weeks); a low-to-moderate dose (less than 20 mg/day of prednisone or equivalent or less than 2 mg/kg/day for a young child); long-term, alternate-day treatment with short-acting preparations; maintenance physiologic doses (replacement therapy); or when administered topically (skin, eyes), inhaled, or by local injection. Health care providers should wait at least 1 month after discontinuation of high dose corticosteroid therapy given for more than 2 weeks before administering the cholera vaccine. Safety and efficacy have not been established in persons with human immunodeficiency virus (HIV) infection; however, according to the opportunistic infection guidelines, adults with CD4 counts greater than 200 cells/mm3 may receive the vaccine if traveling to an area where cholera is epidemic or endemic within the past year.
The cholera vaccine is not absorbed systemically following oral administration and maternal use is not expected to result in fetal exposure to the drug. Maternal cholera disease is associated with adverse pregnancy outcomes including fetal death. The vaccine strain may be shed in the stool of the vaccinated mother for up to 7 days; transmission of the vaccine strain from mother to infant during vaginal delivery is possible. To monitor fetal outcomes of pregnant women exposed to the cholera vaccine, the manufacturer maintains a Vaxchora Pregnancy Registry. The cholera vaccine is not absorbed systemically by the mother following oral administration, and breast-feeding is not expected to result in exposure of the child to the vaccine strain.
The cholera vaccine should never be administered by parenteral administration; it is for oral use only.
The cholera vaccine is not recommended for use in geriatric patients, nor is it approved for use in neonates, infants, or children younger than 2 years of age. The vaccine is only recommended for use by individuals 2 to 64 years of age.

Concurrent administration of the live cholera vaccine with antibiotics active against cholera, such as tetracycline, may diminish vaccine efficacy and result in suboptimal immune response. A duration of fewer than 14 days between stopping antibiotics and vaccination might also be acceptable in some clinical settings if travel cannot be avoided before 14 days have elapsed after stopping antibiotics.
Bismuth Subsalicylate; Metronidazole; Tetracycline: (Major) Avoid the live cholera vaccine in patients that have received tetracycline within 14 days prior to vaccination. Concurrent administration of the live cholera vaccine with antibiotics active against cholera, such as tetracycline, may diminish vaccine efficacy and result in suboptimal immune response. Counsel patients receiving immunosuppressant medications about the possibility of a diminished vaccine response and to continue to follow precautions to avoid exposure to cholera bacteria after receiving the vaccine.
The cholera vaccine contains live attenuated cholera bacteria that replicate in the gastrointestinal tract of the recipient. The exact immunologic mechanism by which the cholera vaccine protects against cholera gastroenteritis is unknown, however, rises in serum vibriocidal antibody 10 days after vaccination with the cholera vaccine were associated with protection in a human challenge study.
The cholera vaccine is administered orally. The vaccine is not absorbed systemically after oral administration.

Oral Route
After oral vaccine administration in adults 18 to 45 years, vaccine efficacy against the occurrence of moderate to severe diarrhea at 10 days post-vaccination was 90.3% (95% CI, 62.7%, 100%) and at 3 months post-vaccination was 79.5% (95% CI, 49.9%, 100%). After a dose of cholera vaccine, peak cholera shedding in stool occurred around day 7. In a study of 53 healthy adults, the percentage of recipients who shed vaccine on day 7 after the dose was 7.5% (95% CI, 2.1%, 18.2%) compared to 11.3% (95% CI, 4.3%, 23%) on any day through 7 days post-vaccination. The duration of shedding of the vaccine strain is unknown.

In the pediatric safety and immunogenicity study (n = 550), classical Inaba vibriocidal antibody seroconversion rates at 10 days post-vaccination in recipients 2 through 17 years of age were comparable to seroconversion rates in adults 18 through 45 years of age (98.5% vs. 93.5%).
