Racepinephrine, a racemic mixture of epinephrine, is an inhalation sympathomimetic bronchodilator agent. Racepinephrine is about one-half as active as epinephrine and the products are labeled for the temporary relief of shortness of breath, chest tightness, and wheezing due to bronchial asthma in adults and children 4 years of age and older. Racepinephrine is about one-half as active as epinephrine. Epinephrine and related epinephrine products have been available for inhalation since the early 1940's. However, concerns over the safety of these products has led to multiple revisions in U.S. Food and Drug Administration (FDA) drug monograph status, including the removal of most non-prescription metered-dose inhaler (MDI) products from the market in 1996. In the United States, racepinephrine is available as a oral inhalation solution (Asthmanefrin, S2 brands) that can be purchased without a prescription (over the counter, or OTC) for use in hand-held rubber bulb nebulizer; these products remain in the final monograph for non-prescription (OTC) bronchodilator drug products and are generally recognized as safe and effective (GRASE). These products have not been evaluated for safety and efficacy via the new drug application (NDA) process, and thus are not approved to be delivered via modern electronic nebulizer or aerolizer devices. The use of non-prescription racepinephrine is generally not recommended in treatment guidelines. Racepinephrine is generally reserved for the acute relief of asthma when selective prescription short-acting beta2-agonists (SABAs) are not available; if symptoms of bronchial asthma are not relieved within 20 minutes or if they become worse, patients should be instructed to discontinue use of the product immediately and seek medical assistance. Excessive use of racepinephrine may result in nervousness, tachycardia, and possibly, other adverse cardiac effects; adverse events subsequent to the marketing of these OTC products have been reported to the FDA. The FDA is reexamining the use of OTC bronchodilator drugs in hand-held rubber bulb nebulizers; the FDA has recommended that these inhalational products be removed from the OTC monograph.

-Racepinephrine is used in an aqueous solution at a concentration equivalent to 1 percent epinephrine.
-Add 0.5 mL (contents of one vial) of racepinephrine solution to the hand-held rubber bulb nebulizer.
-The use of this product by children should be supervised by an adult.
-During clinical use, heart rate and blood pressure monitoring may be advised, as heart rate and blood pressure may increase.
-Storage: Protect unopened vials from light and store at cool temperature between 2 and 20 degrees C (36 to 68 degrees F). Do not freeze. The sterile vials of inhalation solution are each individually wrapped in foil pouches which should remain sealed until the time of use; do not use a vial if the foil pouch has been opened. Do not use the product if it is brown in color or if product color is pinkish or darker than slightly yellow, or if the product is cloudy or if a precipitate is noted. The vials do not contain a preservative. Discard any opened, unused solution.
Racepinephrine, in usual non-prescription use according to the package label, may elevate heart rate and blood pressure. Patients who note an increase in blood pressure or a rapid heart rate are advised to discontinue use of the product and consult their health care provider. The U.S. Food and Drug Administration (FDA) has received multiple adverse event reports since racepinephrine went on the non-prescription market, including complaints of chest pain (unspecified) or angina, nausea and vomiting, increased blood pressure (hypertension), increased heart rate (sinus tachycardia), and patients coughing up pink/red-colored sputum (hemoptysis). These products should only be used in hand-held bulb nebulizers; safety and efficacy in modern electronic or continuous nebulizer machines is not established, and may result in alterations in dosage that may increase the risk for serious adverse reactions, including cardiovascular events. Potential additional cardiovascular adverse events that might be associated with inhalational racepinephrine may include adverse events similar to those seen with systemic epinephrine exposure, such as cardiac arrhythmias or arrhythmia exacerbation, premature ventricular contractions (PVCs), pallor, peripheral vasoconstriction, and palpitations. Rapid rises in blood pressure associated with epinephrine use have produced myocardial infarction, cerebral hemorrhage (intracranial bleeding) and stroke, particularly in elderly patients with cardiovascular disease.
Central nervous system (CNS) side effects of inhalational racepinephrine use can include headache, tremor, and restlessness. Headache can be a symptom of elevated blood pressure in some patients. Dizziness, anxiety, insomnia, panic, psychomotor agitation, tingling, seizures, and weakness have been neurologic and psychiatric reactions reported with systemic epinephrine use, and could potentially occur with racepinephrine inhalations. If a patient notices difficulty sleeping, tremors, nervousness, or seizures while using racepinephrine inhalations, they are advised to discontinue use and consult their health care provider.
Transient increases in blood sugar or hyperglycemia may occur in patients with diabetes who are administered inhaled sympathomimetics, such as racepinephrine. Use racepinephrine with caution in patients with diabetes mellitus. Blood glucose monitoring may be advised.
Racepinephrine should not be used without a prescription (over the counter, or OTC) unless the patient has been told by their health care provider that they have asthma. Also, if the patient has ever been hospitalized for asthma, they are encouraged not to use racepinephrine over-the-counter (OTC), and to seek medical evaluation and prescription instead. Asthma can be a life threatening condition. Signs of asthma exacerbation include: if symptoms are not relieved in 20 minutes, symptoms get worse, the patient needs more than 12 racepinephrine inhalations in 24 hours, the patient uses more than 9 inhalations in 24 hours for more than 3 days a week, or if the patient experiences more than two asthma attacks per week. The patient should consult their health care provider and stop use of racepinephrine if asthma worsens.

Non-selective sympathomimetic agents such as racepinephrine can aggravate a variety of health conditions, and should be used cautiously in patients with diabetes mellitus, pheochromocytoma, closed-angle glaucoma, thyroid disease (especially hyperthyroidism), a psychiatric or emotional condition, a history of seizures, or trouble with urination due to prostatic hypertrophy. Patients should stop use of racepinephrine use if they have insomnia, tremors, nervousness, restlessness, or seizures.

Pregnant women should consult a health care provider prior to the use of non-prescription bronchodilator products such as racepinephrine, as any changes in maternal oxygenation can have an effect on optimal fetal oxygenation. The 2004 guidelines of the National Asthma Education and Prevention Program (NAEPP) Asthma and Pregnancy Working Group include short-acting inhaled beta2-agonists as first line therapy for mild intermittent asthma during pregnancy, and albuterol is preferred due to extensive safety-related information during pregnancy. Due to the potential for interference with uterine contractility and the ability to raise maternal blood pressure and heart rate, use of non-specific sympathomimetic agents, such as racepinephrine, for relief of bronchospasm during labor and obstetric delivery is not recommended.

Consider the benefits of breast-feeding, the risk of potential infant drug exposure, and the risk of an untreated or inadequately treated condition. Alternative FDA-approved agents are available; according to the 2004 recommendations of the National Asthma Education and Prevention Program, short-acting inhaled beta-2 agonists, including albuterol inhalations, may be used during lactation. If a breast-feeding infant experiences an adverse effect related to a maternally ingested drug, healthcare providers are encouraged to report the adverse effect to the FDA.
Children 4 years of age and older should be supervised by an adult during the use of racepinephrine. Racepinephrine should not be used without a prescription in children less than 4 years of age; very young children, infants, or neonates should receive evaluation and advice of a health care provider for the treatment of asthma symptoms. Additionally, administration via a hand-held rubber bulb nebulizer may be challenging in very young children. The FDA has noted that administration via atomizers or mechanical nebulizers is not recommended, as such adminstration may alter the pharmacokinetic, distribution, administered dosage, and safety profile of racepinephrine inhalation solution, and may increase the risk of more serious side effects.

Acarbose: (Moderate) Sympathomimetic agents and adrenergic agonists tend to increase blood glucose concentrations when administered systemically. Monitor for loss of glycemic control when pseudoephedrine, phenylephrine, and other sympathomimetics are administered to patients taking antidiabetic agents. Epinephrine and other sympathomimetics, through stimulation of alpha- and beta- receptors, increase hepatic glucose production and glycogenolysis and inhibit insulin secretion. Also, adrenergic medications may decrease glucose uptake by muscle cells. For treatment of cold symptoms, nasal decongestants may be preferable for short term, limited use (1 to 3 days) as an alternative to systemic decongestants in patients taking medications for diabetes.

Ozanimod: (Contraindicated) Coadministration of ozanimod with racepinephrine is contraindicated. Allow at least 14 days between discontinuation of ozanimod and initiation of racepinephrine. An active metabolite of ozanimod inhibits MAO-B, which may increase the potential for hypertensive crisis. Sympathomimetics such as racepinephrine may increase blood pressure by increasing norepinephrine concentrations and monoamine oxidase inhibitors (MAOIs) are known to potentiate these effects. Patients should be advised to avoid amphetamine drugs, decongestants (including nasal decongestants) and sympathomimetic anorexiants for weight loss, including dietary supplements. Intravenous vasopressors may be used in the emergency management of pulmonary hypertension patients when needed, but hemodynamic monitoring and careful monitoring of cardiac status are needed to avoid ischemia and other complications.

Sympathomimetics, through stimulation of alpha- and beta- receptors, increase hepatic glucose production and glycogenolysis and inhibit insulin secretion. Also, adrenergic medications may decrease glucose uptake by muscle cells. For treatment of cold symptoms, nasal decongestants may be preferable for short term, limited use (1 to 3 days) as an alternative to systemic decongestants in patients taking medications for diabetes.
Tolcapone: (Major) Drugs known to be metabolized by catechol-O-methyltransferase (COMT), such as racepinephrine inhalations , should be administered cautiously in patients receiving COMT inhibitors like entacapone, tolcapone, or combinations containing these drugs such as carbidopa; levodopa; entacapone. If a patient is taking a COMT-inhibitor, then they should seek health care professional advice prior to the use of racepinephrine. Concomitant use may result in increased heart rate, possibly arrhythmias, and excessive changes in blood pressure. A single 400 mg dose of entacapone given with intravenous epinephrine has produced changes in heart rate and blood pressure. Ventricular tachycardia was noted in one healthy volunteer during an interaction study with epinephrine infusion and oral entacapone administration.
Tranylcypromine: (Contraindicated) In general, sympathomimetics should be avoided in patients receiving MAOIs due to an increased risk of hypertensive crisis. This applies to sympathomimetics including stimulants for ADHD, narcolepsy or weight loss, nasal, oral, and ophthalmic decongestants and cold products, and respiratory sympathomimetics (e.g., beta agonist drugs). Some local anesthetics also contain a sympathomimetic (e.g., epinephrine). In general, medicines containing sympathomimetic agents should not be used concurrently with MAOIs or within 14 days before or after their use.
Treprostinil: (Major) Avoid use of sympathomimetic agents with treprostinil. Sympathomimetics counteract the medications used to stabilize pulmonary hypertension, including treprostinil. Sympathomimetics can increase blood pressure, increase heart rate, and may cause vasoconstriction resulting in chest pain and shortness of breath in these patients. Patients should be advised to avoid amphetamine drugs, decongestants (including nasal decongestants) and sympathomimetic anorexiants for weight loss, including dietary supplements. Intravenous vasopressors may be used in the emergency management of pulmonary hypertension patients when needed, but hemodynamic monitoring and careful monitoring of cardiac status are needed to avoid ischemia and other complications.

Umeclidinium; Vilanterol: (Moderate) Administer sympathomimetics with caution with beta-agonists such as vilanterol. The cardiovascular effects of beta-2 agonists may be potentiated by concomitant use. Monitor the patient for tremors, nervousness, increased heart rate, or other additive side effects.
Vasodilators: (Moderate) Use sympathomimetic agents with caution in patients receiving therapy for hypertension. Patients should be monitored to confirm that the desired antihypertensive effect is achieved. Sympathomimetics can increase blood pressure and heart rate, and antagonize the antihypertensive effects of vasodilators when administered concomitantly. Anginal pain may be induced when coronary insufficiency is present.

Patients should avoid dietary supplements containing ingredients that are reported or claimed to have a stimulant or weight-loss effect, such as ephedrine and ephedra, Ma huang, and phenylpropanolamine.

Epinephrine is a nonselective adrenergic agonist; it stimulates alpha1-, alpha2-, beta1-, and beta2-adrenergic receptors, although the degree of stimulation at these receptors may vary depending on the dose administered (i.e., the circulating concentration of epinephrine at the receptor). Stimulation of beta2-receptors by epinephrine leads to arteriolar vasodilation, bronchial smooth muscle relaxation, and increased glycogenolysis. Subsequent to binding at the adrenergic receptor, the intracellular actions of epinephrine are mediated by cyclic adenosine monophosphate (cAMP). The production of cAMP is augmented by beta-stimulation and attenuated by alpha-stimulation. Epinephrine primarily exerts its relaxant effect on bronchial smooth muscle via stimulation of beta2-receptors. Beta2-stimulation also prevents mast cell secretion of histamine and other autocoids, thus antagonizing its effect on end organs and reversing bronchoconstriction and edema. Furthermore, alpha-stimulation may decrease secretions from the bronchial mucosa, attenuating the development of edema. There is some evidence that racepinephrine is effective as a treatment of some pulmonary conditions such as bronchiolitis in children.
Racepinephrine is a racemic mixture of epinephrine that is administered by oral inhalation. The drug is distributed throughout the body. Epinephrine crosses the placenta but does not penetrate the blood-brain barrier to a great extent. Epinephrine will distribute into breast milk. The pharmacologic activity of epinephrine is rapidly terminated by uptake and metabolism in the synaptic cleft. Circulating drug is metabolized by the enzymes catechol-O-methyltransferase (COMT) and monoamine oxidase (MAO) in the liver and in other tissues. These inactive metabolites are then conjugated to either sulfates or glucuronides and renally excreted. Minimal amounts of the drug are excreted unchanged in the urine.

After inhalation of a usual dose via a hand-held bulb nebulizer, racepinephrine, like epinephrine, produces an onset of action usually within 1 to 5 minutes. Inhalational pharmacokinetics of epinephrine are, like other inhaled medications, determined by factors such as the inhaled particle size, the reliability of the device for delivery, and, the rate of delivery via inhalation. Since racepinephrine is regulated under the non-prescription (OTC) bronchodilator drug monograph via the U.S. Food and Drug Administration, pharmacokinetic characteristics required of a new drug application (NDA) process for inhalational drugs, such as assessments of inhalational reliability, specifications of target dose per inhalation, and delivered dose uniformity have not been tested or documented.
