Nortriptyline Hydrochloride 10mg, Oral capsule

Tobacco

· Severity: Moderate

· Notes for Consumers: Tobacco smoking may reduce the effectiveness of this drug in treating your condition. Tobacco smoke contains substances that speed the removal of this drug from the body, which in theory can make this drug less effective. Alternatively, if you take this drug and suddenly stop Tobacco smoking, you may experience drug-related side effects like confusion, constipation, dizziness, difficulty with urination, dry mouth and eyes or drowsiness. Make sure your prescriber is aware that you smoke Tobacco or if you plan to stop smoking.

· Notes for Professionals: Tobacco smoking has been shown to increase the clearance of TCAs by inducing hepatic microsomal enzymes. Sudden smoking cessation may result in a reduced clearance of TCAs and increased TCA effects, despite the initiation of nicotine replacement products.

Alcohol

· Severity: Moderate

· Notes for Consumers: It is recommended that you limit or avoid Alcoholic drinks while you are taking this medication. Alcohol can affect the mood and interfere with the drug treatment, and may increase the risk of side effects, such as sedation and dizziness. Do not drive or operate machinery if you are drinking alcohol-containing beverages.

· Notes for Professionals: Patients should be cautioned about using alcohol concurrently, since tricyclic antidepressants (TCAs) may exaggerate the CNS depressant response to alcohol, leading to an increase in sedation or psychomotor impairment. In some studies, alcohol has increased the unbound form of the TCA in the blood, which might be related to exagerrated clinical effect.

Marijuana

· Severity: Major

· Notes for Consumers: Do not take Marijuana, Medical and Tricyclic Antidepressants together without talking to your health care provider. Taking these drugs together might increase the risk of high blood pressure or heart rate, nervousness, palpitations, problems with sleep, severe headache, or other side effects.

· Notes for Professionals: Avoid administering marijuana and tricyclic antidepressants together as concurrent use may result in adverse cardiovascular effects, such as tachycardia and cardiac arrhythmias. Marijuana is known to produce significant increases in heart rate and cardiac output lasting for 2 to 3 hours. Further, rare case reports of myocardial infarction and cardiac arrhythmias have been associated with marijuana use. Tricyclic antidepressants have also been reported to produce a wide range of cardiovascular effects including cardiac arrhythmias, palpitations, hypertension, ventricular tachycardia, heart failure, and stroke. Coadministration of marijuana with tricyclic antidepressants may result in significant cardiovascular adverse events and thus, should be avoided.
