  • Posaconazole 40mg/1mL, Oral suspension

    Interaction: Alcohol
    Severity: Major
    Notes for Consumers: Some forms of this medication may have other interactions. Discuss the form you are taking with your care team. Drinking alcohol may alter the effects of your medication. Serious side effects may occur. Contact your care team if you experience new or worsening side effects.
    Notes for Professionals: Advise patients that administration of the posaconazole delayed-release oral suspension with alcohol is not recommended. Posaconazole was found to release faster from the delayed-release oral suspension in the presence of alcohol in vitro, which may interfere with its delayed release characteristics.
  • Posaconazole 40mg/1mL, Oral suspension

    Interaction: Cannabis
    Severity: Major
    Notes for Consumers: Discuss cannabis use with your care team. The effects of cannabis may be altered if used with your medication. Call emergency services right away if you experience slow or shallow breathing, shortness of breath, feeling faint, dizziness, confusion, or trouble staying awake.
    Notes for Professionals: Advise patients to avoid cannabis use during posaconazole treatment. Concomitant use may alter the exposure of some cannabinoids and increase the risk for adverse reactions. The cannabinoids delta-9-tetrahydrocannabinol (THC) and cannabidiol (CBD) are CYP3A substrates and posaconazole is a strong CYP3A inhibitor. Concomitant use of a cannabinoid product containing THC and CBD at an approximate 1:1 ratio with another strong CYP3A inhibitor increased THC, 11-OH-THC, and CBD peak exposures by 1.3-, 3-, and 1.9-fold respectively.

    Interaction: Grapefruit juice

    Notes for Consumers: Side effects from posaconazole may result if given with grapefruit juice. Theoretically, grapefruit juice can alter the elimination of posaconazole from the body, and can lead to increased side effects. Your prescriber may decide that taking these together is beneficial for you based on your health conditions. Discuss your drug regimen and health conditions with your health care provider.
    Notes for Professionals: Posaconazole and grapefruit juice should be coadministered with caution due to a potential for altered posaconazole plasma concentrations. Both posaconazole and grapefruit juice are inhibitors of the drug efflux protein, P-glycoprotein, for which posaconazole is a substrate. This interaction may cause alterations in the plasma concentrations of posaconazole, ultimately resulting in an increased risk of adverse events.
