CLOZAPINE/Clozaril (KLOE za peen) treats schizophrenia. It is prescribed when other medications have not worked or cannot be tolerated. It works by balancing the levels of dopamine and serotonin in your brain, substances that help regulate mood. It belongs to a group of medications called antipsychotics.

Caffeine-containing Foods/Beverages

· Severity: Major

· Notes for Consumers: You should limit Caffeine use while taking Clozapine. This includes limiting caffeine intake in foods (examples coffees, colas, teas, and chocolate) as well as in medicines, herbal supplements, and "stay-awake" products. Side effects from Clozapine may get worse if you take Caffeine. Report any troublesome side effects from Clozapine promptly, such as confusion, dry mouth or drooling, headache, drowsiness, dizziness, feeling faint, falls or palpitations. If you notice any unusual changes in mood or ease of movement, contact your prescriber.

· Notes for Professionals: Caffeine may inhibit clozapine metabolism via CYP1A2. Clozapine clearance has been decreased by roughly 14 percent during coadministration of caffeine, and a documented increase in clozapine serum concentrations has occurred in selected patients. In addition, a single case report associates the appearance of psychiatric symptoms with caffeine ingestion in one patient taking clozapine. Until more data are available, caffeine consumption should be minimized during clozapine treatment.

Cannabis

· Severity: Major

· Notes for Consumers: Discuss cannabis use with your care team. The effects of your medication may be altered if used with cannabis. Call emergency services right away if you experience slow or shallow breathing, shortness of breath, feeling faint, dizziness, confusion, or trouble staying awake.

· Notes for Professionals: Advise patients to avoid cannabis use during clozapine treatment due to decreased exposure of clozapine which may alter its efficacy. Concomitant use may also increase the risk for additive CNS depression and other adverse reactions. Cannabis use induces CYP1A2 and clozapine is a CYP1A2 substrate. The induction potential of cannabis is greatest with chronic inhalation. Other routes of administration or sporadic use may have less of an effect.

Tobacco

· Severity: Major

· Notes for Consumers: Do not smoke or use tobacco products while taking this medication. When used together, your medication may not work as well for you.

· Notes for Professionals: Advise patients to avoid tobacco products while receiving clozapine. Tobacco smoking may decrease the plasma concentration of clozapine; monitor for decreased effectiveness. Consider increasing the clozapine dose if necessary. Smoking tobacco induces CYP1A2 and clozapine is a CYP1A2 substrate.

Alcohol

· Severity: Major

· Notes for Consumers: Do not drink alcohol while taking this medication. Drinking alcohol while taking this medication can cause serious side effects, such as central nervous system (CNS) depression, and can increase the risk of falling. Call emergency services right away if you experience symptoms of CNS depression: slow or shallow breathing, shortness of breath, feeling faint, dizziness, confusion, trouble staying awake.

· Notes for Professionals: Advise patients to avoid alcohol consumption while taking CNS depressants. Alcohol consumption may result in additive CNS depression.
