VORICONAZOLE (Generic for VFEND) Lifestyle Interactions

Voriconazole 200mg, Oral tablet

Cannabis

· Severity: Major

· Notes for Consumers: Discuss cannabis use with your care team. The effects of cannabis may be altered if used with your medication. Call emergency services right away if you experience slow or shallow breathing, shortness of breath, feeling faint, dizziness, confusion, or trouble staying awake.

· Notes for Professionals: Advise patients to avoid cannabis use during voriconazole treatment. Concomitant use may alter the exposure of some cannabinoids and increase the risk for adverse reactions. The cannabinoids delta-9-tetrahydrocannabinol (THC) and cannabidiol (CBD) are CYP3A and CYP2C9 substrates and voriconazole is a strong CYP3A and moderate CYP2C9 inhibitor. Concomitant use of a cannabinoid product containing THC and CBD at an approximate 1:1 ratio with another strong CYP3A inhibitor increased THC, 11-OH-THC, and CBD peak exposures by 1.3-, 3-, and 1.9-fold respectively.

Voriconazole 200mg, Oral tablet

Food

· Severity: Mild

· Notes for Consumers: Take oral voriconazole one hour before or one hour after meals. Food may decrease the amount of voriconazole that is absorbed into your body.

· Notes for Professionals: Food may decrease the oral absorption of voriconazole; therefore, oral formulations of voriconazole should be taken 1 hour before or 1 hour after meals.

Voriconazole 200mg, Oral tablet

Grapefruit juice

· Severity: Mild

· Notes for Consumers: Side effects from Voriconazole may get worse if you drink grapefruit juice. Grapefruit juice may decrease the elimination of Voriconazole from your body. If possible, avoid drinking grapefruit juice while you are taking Voriconazole therapy. You may drink other types of juice, such as orange juice.

· Notes for Professionals: Grapefruit juice may interact with voriconazole, although human data are lacking. In the murine model, grapefruit juice administration lead to increases in voriconazole serum concentrations. The mechanism is most likely inhibition of voriconazole metabolism by CYP3A4. Until further studies are available, it may be prudent to advise patients to avoid consumption of grapefruit juice during voriconazole therapy.
