Tapentadol is an oral opioid agonist. Immediate-release tapentadol is indicated for pain severe enough to require an opioid analgesic and for which alternative treatments are inadequate in adults and pediatric patients 6 years and older. The extended-release tablets are indicated for adults with severe pain and neuropathic pain associated with diabetic peripheral neuropathy requiring daily, around-the-clock, long-term opioid treatment and for which alternative treatments are inadequate. Tapentadol activates the mu-opioid receptor and inhibits norepinephrine synaptic reuptake. Norepinephrine reuptake inhibition appears to have additive analgesic effect. Immediate-release tapentadol was effective in treating chronic pain conditions and postsurgical acute pain in adults during clinical trials. Extended-release tapentadol demonstrated significant improvements in pain intensity over 15 weeks in patients with moderate to severe chronic low back pain and better gastrointestinal tolerability compared to controlled-release oxycodone. Use of tapentadol in pediatric patients (6 years and older) with moderate to severe acute postoperative pain resulted in significant reductions in supplemental opioid analgesic medications (morphine equivalents in mg/kg body weight) during the first 12 to 24 hours after the first dose. Tapentadol may be a preferred analgesic in patients with altered cytochrome P450 (CYP450) activity or in adults with mild hepatic or mild to moderate renal impairment because no metabolic activation is necessary for analgesia and no active metabolites have been identified. Use is not recommended in adults with severe hepatic or renal impairment and in pediatric patients with renal or hepatic impairment. Tapentadol may cause serious respiratory depression and expose users to risks of addiction, abuse, and misuse, which can lead to overdose and death. Individualization of dosage regimens and close patient monitoring for adverse effects are needed.

Oral Solid Formulations
Immediate-release tablets
-Avoid use of the immediate-release tablets in pediatric patients who are unable to swallow tablets or who weigh less than 40 kg. The recommended dose cannot be achieved with available tablet strengths in patients who weigh less than 40 kg. Consider the use of tapentadol oral solution in such patients.
Extended-release tablets
-Swallow tablets whole. Do not dissolve, crush, chew, split, or otherwise break as this could lead to the rapid release and absorption of a potentially toxic dose of tapentadol.
-Take each tablet with enough water to ensure complete swallowing immediately after placing in the mouth.
Oral Liquid Formulations
Oral solution
-No dilution is necessary.
-Measure and administer the oral solution using a calibrated measuring device. For adults, dispense the solution with the enclosed oral syringe and adapter. For pediatric patients, dispense with a 3 mL oral syringe for doses that are 3 mL or less or dispense with a 5 mL oral syringe for doses greater than 3 mL.
-The oral solution must be administered to a pediatric patient by an adult. Pediatric patients must not have access to the oral solution.
Pharmacologic tolerance to the analgesic effects of opioid agonists including tapentadol has been reported in some patients. Tolerance is the need for increasing opioid doses to maintain initial pain relief. Typically, tolerance presents as a decrease in the duration of analgesia and is managed by increasing the opioid dose or frequency. When increasing doses of analgesia are required, causes may be multi-factorial including tolerance, progression of disease, or psychological distress.
Tapentadol may cause a variety of effects on the GI system. In premarketing clinical studies of tapentadol, patients taking immediate-release or extended-release, respectively, experienced nausea (30%, 21% to 27%), vomiting (18%, 8% to 12%), xerostomia (4%, 7%), anorexia (2%, 2% to 6%), impaired gastric emptying (less than 1% both), and dyspepsia (2%, 1% to 3%). The incidence of nausea and vomiting increased with increasing dosage in pooled data of immediate-release tapentadol premarketing studies. Diarrhea has been observed in 7% of patients taking extended-release tapentadol and also in postmarketing use. Constipation, due to opioid-induced decreases in GI motility and secretions, has been reported in 8% and 13% to 17% of study patients who received the immediate-release and extended-release tablets, respectively. Abdominal discomfort was reported in 1% of patients taking extended-release tapentadol. Although not specifically reported with tapentadol use, ileus or GI obstruction may result. The GI system rarely acclimates to this effect; therefore, advise patients to follow a bowel regimen consistently during opioid therapy. Increased fluid and fiber intake and the use of stool softeners and laxatives are recommended. Treat persistent symptoms of constipation as required to prevent impaction. Active-controlled trial data suggest that tapentadol may have a greater gastrointestinal tolerability as compared to other opioid agonists. In a trial published in abstract form, the incidence of nausea was 40.9% among those taking tapentadol 200 mg and 57.1% in patients on morphine 60 mg; however, the incidence of vomiting was similar among the treatment groups. In patients taking tapentadol immediate-release 50 to 100 mg compared to those taking oxycodone immediate-release 10 to 15 mg, the incidences of nausea (18.4% vs. 29.4%, respectively), vomiting (16.9% vs. 30%, respectively), and constipation (12.8% vs. 27.1%, respectively) were consistently lower among those taking tapentadol. During a clinical trial in postsurgical pediatric patients who received 1 dose (n = 129) or multiple doses (n = 119) of tapentadol oral solution for moderate to severe acute pain, nausea (1.9%) and vomiting (1.3%) were common reasons for treatment discontinuation. In the multiple dose study in patients 6 years and older, vomiting (24.7%), constipation (16.5%), and nausea (10.6%) were reported.
Tapentadol's effect on opioid receptors in the reticular activating system and striatum produces sedation, drowsiness, or somnolence, reported in 15% and 1% to 14% of study patients on immediate-release and extended-release tablets, respectively. Advise patients that activity requiring mental alertness can be affected due to opioid-induced CNS depression. Confusion (1% immediate-release, less than 1% extended-release) and dizziness (24%, 17% to 18%) have been noted. Patients receiving immediate-release or extended-release tapentadol, respectively, have experienced lethargy (1%, 2%), asthenia (not reported, 2%), fatigue (3%, 9%), hypoesthesia (less than 1%, 1%), paresthesias (less than 1% both), headache (postmarketing reports, 10% to 15%), dysarthria (less than 1% both), abnormal coordination/balance or psychomotor impairment (less than 1% both), nervousness (less than 1%, 1%), irritability (less than 1%, 2%), agitation (less than 1% both), insomnia (2%, 4%), abnormal dreams (1%, 2%), memory impairment (less than 1% both), impaired cognition (not reported, less than 1%), attention disturbance (less than 1%, 1%), disorientation (less than 1% both), anxiety (1%, 2% to 5%), abnormal thinking (less than 1% both), tremor (1%, 1% to 3.4%), chills (not reported, 1% to 1.3%), feeling cold (not reported, 1.3%), and restlessness (less than 1%, not reported). Depression (1%) and depressed mood (1.2%) have been noted in patients taking extended-release tablets; other alterations in feelings experienced in less than 1% of study patients include euphoria and feeling drunk, relaxed, or otherwise abnormal perception disturbances, and nightmares. Hallucinations and panic attack have been reported with postmarketing use. Excitatory symptoms may be indicative of excess serotonin that may progress to a life-threatening syndrome; monitor and evaluate as appropriate. High doses are excitatory due to an increase in circulating catecholamine concentrations; in premarketing trials, seizures were reported in 1 patient with an undisclosed history of epilepsy after a single dose of tapentadol. During a clinical trial in postsurgical pediatric patients who received 1 dose (n = 129) or multiple doses (n = 119) of tapentadol oral solution for moderate to severe acute pain, somnolence (1.9%) was a common reason for treatment discontinuation.
As with other opioid agonists, the most significant adverse effect associated with tapentadol is respiratory depression, reported in less than 1% of study patients. Respiratory depression is more common in elderly or debilitated patients, after large initial doses in non-opioid tolerant patients, when opioids are given with other agents that cause CNS depression, or as a result of sub-acute overdose over an extended period of time. The sensitivity of the respiratory center usually returns to normal within 2 to 3 hours. When opioids are appropriately titrated, the risk of respiratory depression is generally small as tolerance rapidly develops to this effect. Caution needs to be employed in patients with chronic obstructive pulmonary disease, decreased pulmonary reserve, cardiovascular disease, or who are taking other CNS depressants. Treat symptomatic respiratory depression cautiously with an opioid antagonist such as naloxone; acute abstinence syndrome may be precipitated following use of an antagonist in patients who are physically dependent. Hypoxia (less than 1%, not reported), cough (less than 1%, not reported), and dyspnea (less than 1%, 1%) have been reported in patients on immediate-release and extended-release tapentadol, respectively, in separate premarketing trials.
Tapentadol can cause cardiovascular adverse reactions. These events may be symptomatic of a hyperserotonergic state, particularly tachycardia and labile blood pressure, or may occur independently; assess and monitor as appropriate. In separate premarketing trials of immediate-release and extended-release tablets, flushing or hot flush has been reported in 1% and 2% to 3% of patients, respectively. Vertigo was reported in 1% to 2% of patients with extended-release tapentadol. Less than 1% of study patients reported bradycardia, sinus tachycardia, syncope, presyncope, palpitations, and left bundle-branch block. Hypotension (1%) and decreased blood pressure (less than 1%) have been reported. Tapentadol may cause severe hypotension including orthostatic hypotension and syncope in ambulatory patients as a result of peripheral vasodilation. Patients at higher risk of hypotension include those with hypovolemia or those taking concurrent products that compromise vasomotor tone (e.g., phenothiazines, general anesthetics). Monitor patients carefully. In cases of severe respiratory and/or circulatory depression, shock and cardiac arrest may occur.
Urinary system effects reported in less than 1% of patients who received tapentadol include increased urinary frequency (pollakiuria) and urinary hesitation. Urinary retention may occur, particularly if coadministered with anticholinergic agents.
Tapentadol is a controlled substance with a high potential for abuse and psychological dependence. Abuse and addiction are separate and distinct from physiological dependence and tolerance, which can develop during chronic opioid therapy. Tolerance is characterized by a reduced response to a drug after repeated administration. Physiological dependence develops as a result of repeated drug use. It manifests with a withdrawal syndrome after abrupt discontinuation or significant dose reduction, or administration of an agent with opioid antagonist activity (e.g., naloxone). Withdrawal is characterized by restlessness, lacrimation, rhinorrhea, yawning, perspiration, chills, myalgia, and mydriasis. Other symptoms include irritability, anxiety, backache, joint pain, weakness, abdominal cramps, insomnia, nausea, anorexia, vomiting, diarrhea, or increased blood pressure, respiratory rate, or heart rate. Withdrawal has been reported in 1% or less of subjects during clinical trials with tapentadol. Infants born to mothers physically dependent on opioids will also be physically dependent and may exhibit respiratory difficulties and withdrawal. Neonatal opioid withdrawal syndrome may be life-threatening and requires management according to protocols developed by neonatology experts. It presents as irritability, hyperactivity and abnormal sleep pattern, high-pitched cry, tremor, vomiting, diarrhea, and failure to gain weight.
Tapentadol and other opioid agonists are known to cause adverse dermal effects. In clinical trials, generalized pruritus was reported in 3% of patients with immediate-release tablets and 1% of patients with extended-release tablets; pruritus was reported in 5% to 8% and rash (unspecified) in 1% of patients receiving either immediate-release or extended-release tablets. Opioid agonist-induced pruritus is thought to be mediated through stimulation of central opioid receptors since opioid antagonists (i.e., nalmefene or naloxone) have been shown to ameliorate pruritus associated with cholestasis. Use of H1-blockers or changing to a different opioid may lessen the pruritus and/or rash. In addition, hyperhidrosis was reported by 3% of study patients on immediate-release and 3% to 5% on extended-release tapentadol. Pruritus (9.4%) was reported during a clinical trial in postsurgical pediatric patients 6 years and older who received multiple doses of tapentadol oral solution for moderate to severe acute pain (n = 119).
Drug hypersensitivity (reaction undefined) and urticaria have occurred in less than 1% of patients in premarketing clinical trials of tapentadol. Anaphylactoid reactions, including cases of anaphylaxis, angioedema, and anaphylactic shock have been noted very rarely with postmarketing use. Advise patients how to recognize such reactions and when to seek medical treatment.
Serotonin syndrome has been reported in patients taking opioids at recommended doses. Patients taking opioids concomitantly with a serotonergic medication should seek immediate medical attention if they develop symptoms such as agitation, hallucinations, tachycardia, fever, excessive sweating, shivering or shaking, muscle twitching or stiffness, trouble with coordination, nausea, vomiting, or diarrhea. Symptoms generally present within hours to days of taking an opioid with another serotonergic agent, but may also occur later, particularly after a dosage increase. If serotonin syndrome is suspected, either the opioid and/or the other agent should be discontinued.
Upper respiratory tract infection, urinary tract infection, and naso-pharyngitis have each been reported with greater frequency among study patients taking immediate-release tapentadol (1%) as compared to those taking placebo (less than 1%); however, causality to the drug has not been established.
Eye disorders have been reported in study patients receiving tapentadol, including blurred vision (1%) with extended-release tablets and undefined visual disturbance (less than 1%) with both formulations.
Opioid agonists can interfere with the endocrine system by inhibiting the secretion of adrenocorticotropic hormone (ACTH), cortisol, and luteinizing hormone (LH), and by stimulating secretion of prolactin, growth hormone (GH), insulin, and glucagon. Chronic opioid use may influence the hypothalamic-pituitary-gonadal axis, leading to hormonal changes that may manifest as hypogonadism (gonadal suppression). Although the exact causal role of opioids in the clinical manifestations of hypogonadism is unknown, patients could experience libido decrease, impotence, amenorrhea, or infertility. Of patients on extended-release tapentadol, impotence (erectile dysfunction) was reported in 1% of study patients, and undefined sexual dysfunction has been reported with post-marketing use. Other various medical, physical, lifestyle, and psychological stressors may influence gonadal hormone concentrations; these stressors have not been adequately controlled for in clinical studies with opioids. Patients presenting with signs or symptoms of androgen deficiency should undergo laboratory evaluation. Opioid agonists can inhibit the release of thyrotropin, leading to a decrease in thyroid hormone. Morphine and related compounds can stimulate the release of vasopressin (ADH). Hyponatremia can occur as a result of SIADH.
Opioids may interfere with the endocrine system by inhibiting the secretion of adrenocorticotropic hormone (ACTH) and cortisol. Rarely, adrenocortical insufficiency has been reported in association with opioid use. Patients should seek immediate medical attention if they experience symptoms such as nausea, vomiting, loss of appetite, fatigue, weakness, dizziness, or hypotension. If adrenocortical insufficiency is suspected, confirm with diagnostic testing as soon as possible. If diagnosed, the patient should be treated with physiologic replacement doses of corticosteroids, and if appropriate, weaned off of opioid therapy. If the opioid can be discontinued, a follow-up assessment of adrenal function should be performed to determine if corticosteroid treatment can be discontinued. Other opioids may be tried; some cases reported use of a different opioid with no recurrence of adrenocortical insufficiency. It is unclear which, if any, opioids are more likely to cause adrenocortical insufficiency.
Weight loss has been noted in less than 1% of study patients receiving extended-release tapentadol; this effect has not been specifically reported with immediate-release use.
Suicidal ideation has been reported in postmarketing experience with tapentadol.
Cases of opioid-induced hyperalgesia (OIH) have been reported, both with short-term and longer-term use of opioids. OIH occurs when an opioid paradoxically causes an increase in pain or an increase in sensitivity to pain. Symptoms of OIH include increased levels of pain upon opioid dosage increase, decreased levels of pain upon opioid dosage decrease, or pain from ordinarily non-painful stimuli (allodynia). These symptoms may suggest OIH only if there is no evidence of underlying disease progression, opioid tolerance, opioid withdrawal, or addictive behavior. Data suggests a strong biologic plausibility between opioids and OIH and allodynia. If OIH is suspected, carefully consider appropriately decreasing the dose of the current opioid analgesic or opioid rotation (safely switching to a different opioid).
Fever (5.9%) was reported during a clinical trial in postsurgical pediatric patients 6 years and older who received multiple doses of tapentadol oral solution for moderate to severe acute pain (n = 119).
Hypoglycemia has been reported during opioid therapy. Most reports occurred in persons with at least 1 predisposing risk factor, such as diabetes.
Tapentadol is contraindicated for use in patients with a known hypersensitivity to the drug or any of its ingredients. Cases of anaphylaxis and angioedema have been reported with postmarketing use of the drug.
Opioid use requires an experienced clinician who is knowledgeable about the use of opioids, including the use of extended-release/long-acting opioids, and how to mitigate the associated risks. Opioids expose users to the risks of addiction, abuse, and misuse, which can occur at any dosage or duration. Although the risk of addiction in any individual is unknown, it can occur in persons appropriately prescribed opioids. Addiction can occur at recommended dosages and if the drug is misused or abused. Assess each individual's risk for opioid addiction, abuse, or misuse before prescribing an opioid, and monitor for the development of these behaviors or conditions. Risks are increased in persons with a personal or family history of substance abuse (including alcoholism) or mental illness (e.g., major depression). The potential for these risks should not prevent the proper management of pain in any given individual. Persons at increased risk may be prescribed opioids, but use in such persons necessitates intensive counseling about the risks and proper use of the opioid along with intensive monitoring for signs of addiction, abuse, and misuse. Abuse and addiction are separate and distinct from physical dependence and tolerance; persons with addiction may not exhibit tolerance and symptoms of physical dependence. Opioids are sought by drug abusers and persons with addiction disorders and are subject to criminal diversion. Abuse of opioids has the potential for overdose or poisoning and death. Consider these risks when prescribing or dispensing an opioid. Strategies to reduce these risks include prescribing the drug in the smallest appropriate quantity. Abuse or misuse of tapentadol extended-release tablets by cutting, breaking, chewing, crushing, snorting, or injecting the dissolved product will result in the uncontrolled delivery of tapentadol and can result in overdose and death. Consumption of tapentadol extended-release tablets with ethanol will result in increased drug exposure that may cause a fatal overdose; advise against ethanol ingestion and ethanol intoxication, including the ingestion of alcohol contained in prescription or non-prescription medications. Dosing errors may result from confusion between mg and mL when prescribing, dispensing, and administering tapentadol oral solution. Ensure that the dose is communicated clearly and dispensed accurately. Instruct patients on how to measure the dose and to use a calibrated oral dosing device. Keep opioids out of the reach of pediatric persons, others for whom the drug was not prescribed, and pets as accidental exposure or improper use may cause respiratory failure and a fatal overdose. Accidental exposure of even a single dose of an opioid, especially by younger persons, can result in a fatal overdose. Because the risk of overdose increases as opioid dose increases, reserve titration to higher doses of an opioid for persons in whom lower doses are insufficiently effective and in whom the expected benefits of using a higher dose opioid clearly outweigh the substantial risks. Do not use immediate-release opioids for an extended period unless the pain remains severe enough to require an opioid and for which alternative treatment options continue to be inadequate. Many acute pain conditions (e.g., pain occurring with surgical procedures or acute musculoskeletal injuries) require no more than a few days of an opioid. Clinical guidelines on opioid prescribing for some acute pain conditions are available. Extended-release opioids are not intended for use in the management of acute pain or on an as-needed basis but rather only for the management of severe and persistent pain that requires an extended treatment period with a daily opioid and for which alternative treatment options are inadequate. Discuss the availability of naloxone with all persons and consider prescribing it in persons who are at increased risk of opioid overdose, such as persons who are also using other CNS depressants, who have a history of opioid use disorder (OUD), who have experienced a previous opioid overdose, or who have household members or other close contacts at risk for accidental exposure or opioid overdose.
Tapentadol is contraindicated in persons with significant respiratory depression and those with acute or severe asthma in an unmonitored setting or in the absence of resuscitative equipment. Avoid coadministration with other CNS depressants when possible, as this significantly increases the risk for profound sedation, respiratory depression, coma, and death. Reserve concomitant prescribing of these drugs for use in persons for whom alternative treatment options are inadequate; if concurrent use is necessary, use the lowest effective dosages and minimum treatment durations needed. Monitor closely for signs or symptoms of respiratory depression and sedation. Persons with chronic obstructive pulmonary disease (COPD), cor pulmonale, respiratory insufficiency, hypoxemia, hypercapnia, or preexisting respiratory depression are at increased risk of decreased respiratory drive even at recommended doses. Persons with advanced age, cachexia, or debilitation are also at an increased risk for opioid-induced respiratory depression. Monitor such persons closely, particularly when initiating and titrating the opioid; consider the use of non-opioid analgesics. Opioids increase the risk of central sleep apnea (CSA) and sleep-related hypoxemia in a dose-dependent fashion. Consider decreasing the opioid dosage in persons with CSA. Respiratory depression, if left untreated, may cause respiratory arrest and death. Carbon dioxide retention from respiratory depression may also worsen opioid sedating effects. Management of respiratory depression may include observation, necessary supportive measures, and opioid antagonist use when indicated.
To reduce the risk of life-threatening adverse effects in patients taking tapentadol extended-release tablets, do not exceed an initial dose of 50 mg every 12 hours in opioid-naive patients. Further, extended-release tapentadol should not be used for the treatment of mild pain or pain that is not expected to persist for an extended period of time, acute pain, or as an as-needed (prn) analgesic. Reserve use of the extended-release tablets for patients in whom alternative treatment options (e.g., non-opioid analgesics or immediate-release opioids) are ineffective, not tolerated, or would be otherwise inadequate to provide sufficient management of pain.
Tapentadol is contraindicated in persons with known or suspected GI obstruction, including paralytic ileus. Tapentadol may cause spasm of the sphincter of Oddi. Opioids may cause increases in serum amylase. Monitor persons with biliary tract disease, including acute pancreatitis, for worsening symptoms.
Tapentadol use is contraindicated in patients who are receiving or have received MAOI therapy within the past 14 days. Additive CNS depression, drowsiness, dizziness, or hypotension may occur. Concomitant use may increase the risk for serotonin syndrome.
Any patient receiving an opiate agonist should be warned about the possibility of sedation and to use caution when driving or operating machinery.
Avoid tapentadol use in persons with CNS depression, impaired consciousness, or coma; opioids may obscure the clinical course in a person with a head trauma injury. Monitor persons who may be susceptible to the intracranial effect of carbon dioxide retention (e.g., those with evidence of increased intracranial pressure, brain tumor, or intracranial mass) for signs of sedation and respiratory depression, particularly when initiating tapentadol therapy. Tapentadol may reduce respiratory drive and resultant carbon dioxide retention can further increase intracranial pressure.
Seizures can be precipitated by opioid agonists in patients with a preexisting seizure disorder. The incidence of these effects during tapentadol therapy is not known, but appears to be rare at normal doses. Monitor patients with a history of seizure disorders for worsened seizure control during therapy.
Avoid abrupt discontinuation of tapentadol in a physically-dependent patient. Taper the dose gradually while monitoring carefully for signs and symptoms of withdrawal. If the patient develops these signs or symptoms, raise the dose to the previous level and taper more slowly, either by increasing the interval between decreases, decreasing the amount of change in dose, or both. When discontinuing tapentadol therapy in patients who are opioid-dependent, taper the dose gradually by 10% to 25% every 2 to 4 weeks. Patients who have been taking opioids for briefer periods of time may tolerate a more rapid taper. Ensure a multimodal approach to pain management, including mental health support if needed, is in place prior to an opioid analgesic taper. Consider tapering to reduced opioid dosage, or tapering and discontinuing long-term opioid therapy, when pain improves; the patient requests dosage reduction or discontinuation; pain and function are not meaningfully improved; the patient is receiving higher opioid doses without evidence of benefit from the higher dose; the patient has current evidence of opioid misuse; the patient experiences side effects that diminish quality of life or impair function; the patient experiences an overdose or other serious event (e.g., hospitalization, injury) or has warning signs for an impending event such as confusion, sedation, or slurred speech; the patient is receiving medications (e.g., benzodiazepines) or has medical conditions (e.g., lung disease, sleep apnea, liver disease, kidney disease, fall risk, advanced age) that increase risk for adverse outcomes; or the patient has been treated with opioids for a prolonged period and current benefit-harm balance is unclear. If the patient has a serious mental illness, is at high suicide risk, or has suicidal ideation, offer or arrange for consultation with a behavioral health provider before initiating a taper. In patients with opioid use disorder, offer or arrange for medication-assisted treatment. Advise patients that there is an increased risk for overdose on abrupt return to a previously prescribed higher dose; provide opioid overdose education, and consider offering naloxone. Monitor patients closely for anxiety, depression, suicidal ideation, and opioid use disorder, and offer support and referral as needed.
Use tapentadol with caution in adults with hepatic disease. Higher serum concentrations have been detected in patients with hepatic impairment compared to those detected in patients with normal hepatic function. Tapentadol undergoes extensive inactivation via first pass metabolism; therefore, greater systemic exposure can be expected with declining hepatic function. Use is not recommended in adults with severe hepatic impairment. Dose adjustment and caution are recommended if tapentadol is used in adults with moderate hepatic impairment. Closely observe patients with moderate impairment for respiratory or central nervous system depression during treatment initiation and dose titration. Avoid use of tapentadol in pediatric patients with hepatic impairment.
Avoid use of tapentadol in adults with severe renal impairment or renal failure due to accumulation of a metabolite formed during tapentadol glucuronidation. The clinical relevance of the metabolite is not known. No tapentadol dosing adjustments are required in adults with mild to moderate renal dysfunction. Avoid use of tapentadol in pediatric patients with renal impairment.
Use tapentadol with caution in geriatric or debilitated adults, as these populations are more susceptible to adverse reactions, especially sedation and respiratory depression, probably as a result of altered distribution of the drug or decreased elimination. Initial doses may need to be reduced, and doses should be carefully titrated, taking into account analgesic effects, adverse reactions, and concomitant conditions or drugs that may increase CNS depression and depress respiration. According to the Beers Criteria, opiate agonists are considered potentially inappropriate medications (PIMs) in geriatric adults with a history of falls or fractures; avoid in these patient populations, except in the setting of severe acute pain, since opiates can produce ataxia, impaired psychomotor function, syncope, and additional falls. If an opiate must be used, consider reducing the use of other CNS-active medications that increase the risk of falls and fractures and implement strategies to reduce fall risk. In patients receiving palliative care, the balance of benefits and harms of medication management may differ from those of the general population of older adults.
Available data with tapentadol are insufficient to inform a drug-associated risk for major birth defects and miscarriage. In an animal study, multiple malformations and embryofetal toxicity were noted in pregnant rabbits given tapentadol subcutaneously at a range of doses varying from therapeutic to supratherapeutic. In addition, skeletal development delays, low birth weight, and decreased rates of weight gain were noted in the pups of rats given tapentadol at doses associated with maternal toxicity. Based on animal data, advise pregnant women of the potential risk to the fetus. Tapentadol is not recommended for use during and immediately before labor when other analgesic techniques are more appropriate. Opioids can prolong labor and obstetric delivery by temporarily reducing the strength, duration, and frequency of uterine contractions. This effect is not consistent and may be offset by an increased rate of cervical dilatation, which may shorten labor. Opioids cross the placenta and may produce respiratory depression and psycho-physiologic effects in the neonate. Monitor neonates exposed to opioid analgesics during labor for signs of excess sedation and respiratory depression. An opioid antagonist (e.g., naloxone) should be available for reversal of opioid-induced respiratory depression in the neonate. Further, prolonged maternal use of opioids during pregnancy may result in neonatal opioid withdrawal syndrome (NOWS). Monitor the exposed neonate for withdrawal symptoms, including irritability, hyperactivity and abnormal sleep pattern, high-pitched cry, tremor, vomiting, diarrhea, and failure to gain weight, and manage accordingly. Onset, duration, and severity of opioid withdrawal may vary based on the specific opioid used, duration of use, timing and amount of last maternal use, and rate of elimination by the newborn. Guidelines recommend early universal screening of pregnant patients for opioid use and opioid use disorder at the first prenatal visit. Obtain a thorough history of substance use and review the Prescription Drug Monitoring Program to determine if patients have received prior prescriptions for opioids or other high-risk drugs such as benzodiazepines. Discuss the risks and benefits of opioid use during pregnancy, including the risk of becoming physiologically dependent on opioids, the possibility for NOWS, and how long-term opioid use may affect care during a future pregnancy. In women undergoing uncomplicated normal spontaneous vaginal birth, consider opioid therapy only if expected benefits for both pain and function are anticipated to outweigh risks to the patient. If opioids are used, use in combination with nonpharmacologic therapy and nonopioid pharmacologic therapy, as appropriate. Use immediate-release opioids instead of extended-release or long-acting opioids; order the lowest effective dosage and prescribe no greater quantity of opioids than needed for the expected duration of such pain severe enough to require opioids. For women using opioids for chronic pain, consider strategies to avoid or minimize the use of opioids, including alternative pain therapies (i.e., nonpharmacologic) and nonopioid pharmacologic treatments. Opioid agonist pharmacotherapy (e.g., methadone or buprenorphine) is preferable to medically supervised withdrawal in pregnant women with opioid use disorder.
There is insufficient or limited data on the presence of tapentadol in human milk. Tapentadol is present in animal milk. Available physiochemical and pharmacodynamic or toxicological data with tapentadol point to excretion in breast milk and risk to a breast-feeding child cannot be excluded. Because of the potential for serious adverse reactions including excess sedation and respiratory depression in a breast-fed infant, breast-feeding is not recommended during treatment with extended-release tapentadol. For immediate-release tapentadol, consider the developmental and health benefits of breast-feeding along with the mother's clinical need for tapentadol and any potential adverse effects on the breast-fed infant from tapentadol or the underlying maternal condition. Alternative analgesics considered to be usually compatible with breast-feeding by previous American Academy of Pediatrics recommendations include acetaminophen, ibuprofen, and morphine.
Chronic opioid use may influence the hypothalamic-pituitary-gonadal axis, leading to hormonal changes that may manifest as hypogonadism (gonadal suppression) and pose a reproductive risk. Although the exact causal role of opioids in the clinical manifestations of hypogonadism is unknown, patients could experience libido decrease, impotence, amenorrhea, or infertility. It is not known whether the effects on fertility are reversible. Monitor patients for symptoms of opioid-induced endocrinopathy. Patients presenting with signs or symptoms of androgen deficiency should undergo laboratory evaluation.
Tapentadol may cause severe hypotension, including orthostatic hypotension and syncope in ambulatory persons. There is an increased risk in persons whose ability to maintain blood pressure has already been compromised by hypovolemia or concurrent administration of certain CNS depressant drugs (e.g., phenothiazines, general anesthetics). Monitor for signs of hypotension after initiating or titrating the opioid dosage. Avoid the use of tapentadol in persons with circulatory shock; it may cause vasodilation that can further reduce cardiac output and blood pressure.
General Dosing Information
-Individualize dosing for each patient; consider severity of pain, patient response, prior analgesic treatment experience, and risk factors for addiction, abuse, and misuse. Use the lowest effective dose for the shortest duration consistent with individual treatment goals. Reserve titration to higher doses for patients in whom lower doses are insufficient and in whom expected benefits of using a higher dose opioid clearly outweigh substantial risks.
-Monitor patients closely for respiratory depression, which may occur at any time during therapy. Consider this risk when selecting an initial dosage and when adjusting the dose. Continually reevaluate patients to assess the maintenance of pain control and the relative incidence of adverse reactions, as well as to monitor for the development of addiction, abuse, or misuse. If the level of pain increases after dosage stabilization, attempt to identify the source of increased pain before increasing the opioid dosage. If unacceptable opioid-related adverse reactions are observed, consider reducing the dosage. Adjust the dosage to obtain an appropriate balance between management of pain and opioid-related adverse reactions.
-Many acute pain conditions (e.g., pain occurring with surgical procedures or acute musculoskeletal injuries) require no more than a few days of an opioid. Clinical guidelines on opioid prescribing for some acute pain conditions are available.
-There is substantial interpatient variability in the relative potency of different opioid drugs and products. When adjusting tapentadol dosages or converting from another opioid agonist, it is better to underestimate the patient's daily dose requirements and give rescue doses than to risk an adverse reaction. If an adverse reaction occurs, the next dose may be reduced. Refer to the Opioid Agonists Drug Class Overview for approximate equianalgesic doses.
-When discontinuing tapentadol therapy in patients who are opioid-dependent, taper the dose gradually by 10% to 25% every 2 to 4 weeks. Patients who have been taking opioids for briefer periods of time may tolerate a more rapid taper. Ensure a multimodal approach to pain management, including mental health support if needed, is in place prior to an opioid analgesic taper.
For the treatment of severe pain where treatment with an opioid is appropriate and for which alternative treatments are inadequate:

If a patient receives naltrexone and an opiate agonist is needed for an emergency situation, large doses of opiate agonists may ultimately overwhelm naltrexone antagonism of opiate receptors. Immediately following administration of exogenous opiate agonists, the opiate plasma concentration may be sufficient to overcome naltrexone competitive blockade, but the patient may experience deeper and more prolonged respiratory depression and thus, may be in danger of respiratory arrest and circulatory collapse. Non-receptor mediated actions like facial swelling, itching, generalized erythema, or bronchoconstriction may occur presumably due to histamine release. Coadministration of intravenous morphine decreased the Cmax and AUC of clopidogrel's active metabolites by 34%. Time required for maximal inhibition of platelet aggregation (median 3 hours vs. 1.25 hours) was significantly delayed; times up to 5 hours were reported. Inhibition of platelet plug formation was delayed and residual platelet aggregation was significantly greater 1 to 4 hours after morphine administration.

COMT inhibitors have also been associated with sudden sleep onset during activities of daily living such as driving, which has resulted in accidents in some cases. Prescribers should re-assess patients for drowsiness or sleepiness regularly throughout treatment, especially since events may occur well after the start of treatment.
Crofelemer: (Moderate) Pharmacodynamic interactions between crofelemer and opiate agonists are theoretically possible. Crofelemer does not affect GI motility mechanisms, but does have antidiarrheal effects. Patients taking medications that decrease GI motility, such as opiate agonists, may be at greater risk for serious complications from crofelemer, such as constipation with chronic use. Use caution and monitor GI symptoms during coadministration.

Darifenacin: (Moderate) Monitor patients for signs of urinary retention or reduced gastric motility when darifenacin, an anticholinergic drug for overactive bladder, is used with opiate agonists. The concomitant use of these drugs together may increase risk of urinary retention and/or severe constipation, which may lead to paralytic ileus. Opiates increase the tone and decrease the propulsive contractions of the smooth muscle of the gastrointestinal tract. Prolongation of the gastrointestinal transit time may be the mechanism of the constipating effect. Both agents may also cause drowsiness or blurred vision, and patients should use care in driving or performing other hazardous tasks until the effects of the drugs are known.
Desflurane: (Moderate) Concurrent use with opiate agonists can decrease the minimum alveolar concentration (MAC) of desflurane needed to produce anesthesia.

Opiate analgesics combined with antimuscarinics can cause severe constipation or paralytic ileus, especially with chronic use. Additive CNS effects like drowsiness or dizziness may also occur.

Nalbuphine: (Major) Avoid the concomitant use of tapentadol and opiate agonists/antagonists (e.g., buprenorphine, butorphanol, nalbuphine, or pentazocine). Opiate agonists/antagonists may partially block the analgesic, respiratory depressant, and CNS depressant effects of tapentadol. Due to their antagonistic properties, opiate agonists/antagonists may cause withdrawal symptoms in patients receiving chronic opiate agonists. These agents may also be used concurrently with some opiate agonists and cause additive CNS, respiratory, and hypotensive effects. Consider dose reduction of the opiate agonist in situations of concomitant prescription. The additive or antagonistic effects are dependent upon the dose of pure opiate agonist used; antagonistic effects are more common at low to moderate doses of the pure opiate agonist.
Naltrexone: (Major) When naltrexone is used as adjuvant treatment of opiate or alcohol dependence, use is contraindicated in patients currently receiving opiate agonists. Naltrexone will antagonize the therapeutic benefits of opiate agonists and will induce a withdrawal reaction in patients with physical dependence to opioids. Also, patients should be opiate-free for at least 7-10 days prior to initiating naltrexone therapy. If there is any question of opioid use in the past 7-10 days and the patient is not experiencing opioid withdrawal symptoms and/or the urine is negative for opioids, a naloxone challenge test needs to be performed. If a patient receives naltrexone and an opiate agonist is needed for an emergency situation, large doses of opiate agonists may ultimately overwhelm naltrexone antagonism of opiate receptors. Immediately following administration of exogenous opiate agonists, the opiate plasma concentration may be sufficient to overcome naltrexone competitive blockade, but the patient may experience deeper and more prolonged respiratory depression and thus, may be in danger of respiratory arrest and circulatory collapse. Non-receptor mediated actions like facial swelling, itching, generalized erythema, or bronchoconstriction may occur presumably due to histamine release. A rapidly acting opiate agonist is preferred as the duration of respiratory depression will be shorter. Patients receiving naltrexone may also experience opiate side effects (respiratory insufficiency or arrest or circulatory collapse) with low doses of opiate agonists. If the opiate agonist is taken in such a way that high concentrations remain in the body beyond the time naltrexone exerts its therapeutic effects, serious side effects may occur.
Naratriptan: (Moderate) Because of the potential risk and severity of serotonin syndrome, caution should be observed when administering tapentadol with serotonin-receptor agonists. Inform patients taking this combination of the possible increased risks and monitor for the emergence of serotonin syndrome particularly during treatment initiation and dose adjustment. Consider the use of a parenteral antiplatelet agent in acute coronary syndrome patients requiring an opioid agonist. Mean ticagrelor exposure decreased up to 36% in ACS patients undergoing PCI when intravenous morphine was administered with a loading dose of ticagrelor; mean platelet aggregation was higher up to 3 hours post loading dose. Similar effects on ticagrelor exposure and platelet inhibition were observed when fentanyl was administered with a ticagrelor loading dose in ACS patients undergoing PCI. The anticholinergic properties of tricyclic antidepressants may increase the risk of urinary retention and/or severe constipation, which may lead to paralytic ileus. Trihexyphenidyl: (Moderate) Tapentadol should be used cautiously with anticholinergic medications since additive depressive effects on GI motility or bladder function may occur. Monitor patients for signs of urinary retention or reduced gastric motility. Opiates increase the tone and decrease the propulsive contractions of the smooth muscle of the gastrointestinal tract. Prolongation of the gastrointestinal transit time may be the mechanism of the constipating effect. Opiate analgesics combined with antimuscarinics can cause severe constipation or paralytic ileus, especially with chronic use. Additive CNS effects like drowsiness or dizziness may also occur.
Trimethobenzamide: (Moderate) The concurrent use of trimethobenzamide with other medications that cause CNS depression, like opiate agonists, may potentiate the effects of either trimethobenzamide or the opiate agonist.
Trimipramine: (Major) Concomitant use of opioid agonists with tricyclic antidepressants may cause excessive sedation, somnolence, and increased risk of serotonin syndrome. Trospium: (Moderate) Monitor patients for signs of urinary retention or reduced gastric motility when trospium, an anticholinergic drug for overactive bladder, is used with opiate agonists. The concomitant use of these drugs together may increase risk of urinary retention and/or severe constipation, which may lead to paralytic ileus. Opiates increase the tone and decrease the propulsive contractions of the smooth muscle of the gastrointestinal tract. Prolongation of the gastrointestinal transit time may be the mechanism of the constipating effect. Both agents may also cause drowsiness or blurred vision, and patients should use care in driving or performing other hazardous tasks until the effects of the drugs are known.
Valerian, Valeriana officinalis: (Moderate) Concomitant use of opioid agonists with valerian may cause excessive sedation and somnolence. Limit the use of opioid pain medication with valerian to only patients for whom alternative treatment options are inadequate. If concurrent use is necessary, use the lowest effective doses and minimum treatment durations needed to achieve the desired clinical effect.
Valproic Acid, Divalproex Sodium: (Moderate) Concomitant use of opioid agonists with valproic acid may cause excessive sedation and somnolence. Limit the use of opioid pain medications with valproic acid to only patients for whom alternative treatment options are inadequate. If concurrent use is necessary, use the lowest effective doses and minimum treatment durations needed to achieve the desired clinical effect. Educate patients about the risks and symptoms of excessive CNS depression.
Valsartan; Hydrochlorothiazide, HCTZ: (Moderate) Monitor for decreased diuretic efficacy and additive orthostatic hypotension when thiazide diuretics are administered with tapentadol. Adjustments to diuretic therapy may be needed in some patients. The efficacy of diuretics may be reduced due to opioid-induced release of antidiuretic hormone.
Venlafaxine: (Moderate) If concomitant use of tapentadol and serotonin norepinephrine reuptake inhibitors is warranted, monitor patients for the emergence of serotonin syndrome. Discontinue all serotonergic agents and initiate symptomatic treatment if serotonin syndrome occurs. The concomitant use of opioids with other drugs that affect the serotonergic neurotransmitter system has resulted in serotonin syndrome.
Vigabatrin: (Moderate) Vigabatrin may cause somnolence and fatigue. Drugs that can cause CNS depression, if used concomitantly with vigabatrin, may increase both the frequency and the intensity of adverse effects such as drowsiness, sedation, and dizziness. Caution should be used when vigabatrin is given with opiate agonists.
Vilazodone: (Moderate) Because of the potential risk and severity of excessive sedation, somnolence, and serotonin syndrome, caution should be observed when administering tapentadol with vilazodone. If concurrent use is necessary, use the lowest effective doses and minimum treatment durations needed to achieve the desired clinical effect. Inform patients taking this combination of the possible increased risks and monitor for the emergence of excessive CNS depression and serotonin syndrome, particularly during treatment initiation and dose adjustment. Discontinue all serotonergic agents and initiate symptomatic treatment if serotonin syndrome occurs.
Viloxazine: (Major) Caution is advised when tapentadol is coadministered with selective norepinephrine reuptake inhibitors. The combined use of these drugs may result in excessive concentrations of norepinephrine, increasing the risk of adverse cardiac effects. If concomitant treatment is clinically warranted, careful observation of the patient is advised, especially during initiation of the second therapy and after dosage adjustments (increases) of either agent.
Vortioxetine: (Moderate) Because of the potential risk and severity of serotonin syndrome, caution should be observed when administering tapentadol with vortioxetine. Inform patients taking this combination of the possible increased risks and monitor for the emergence of serotonin syndrome particularly during treatment initiation and dose adjustment. Discontinue all serotonergic agents and initiate symptomatic treatment if serotonin syndrome occurs.
Zaleplon: (Major) Concomitant use of opioid agonists with other CNS depressants may cause excessive sedation and somnolence. Limit the use of opioid pain medications with anotehr CNS depressant to only patients for whom alternative treatment options are inadequate. If concurrent use is necessary, use the lowest effective doses and minimum treatment durations needed to achieve the desired clinical effect. Educate patients about the risks and symptoms of excessive CNS depression.
Ziconotide: (Moderate) Concurrent use of ziconotide and opiate agonists may result in an increased incidence of dizziness and confusion. Ziconotide neither interacts with opiate receptors nor potentiates opiate-induced respiratory depression. However, in animal models, ziconotide did potentiate gastrointestinal motility reduction by opioid agonists.
Ziprasidone: (Moderate) Because of the potential for additive sedation and CNS depression, caution should be observed when administering tapentadol with ziprasidone. Tapentadol is associated with reports of serotonin syndrome when given with other serotonergic drugs; there are case reports of serotonin syndrome with use of ziprasidone postmarketing but causality is not established. Discontinue all serotonergic agents and initiate symptomatic treatment if serotonin syndrome occurs.
Zolmitriptan: (Moderate) Because of the potential risk and severity of serotonin syndrome, caution should be observed when administering tapentadol with serotonin-receptor agonists. Inform patients taking this combination of the possible increased risks and monitor for the emergence of serotonin syndrome particularly during treatment initiation and dose adjustment. Discontinue all serotonergic agents and initiate symptomatic treatment if serotonin syndrome occurs.
Zolpidem: (Major) Concomitant use of opioid agonists with other CNS depressants may cause excessive sedation and somnolence. Limit the use of opioid pain medications with anotehr CNS depressant to only patients for whom alternative treatment options are inadequate. If concurrent use is necessary, use the lowest effective doses and minimum treatment durations needed to achieve the desired clinical effect. Educate patients about the risks and symptoms of excessive CNS depression.
Zuranolone: (Major) Avoid concomitant use of opioids and other CNS depressants, such as zuranolone. Concomitant use can increase the risk of respiratory depression, hypotension, profound sedation, and death. If alternate treatment options are inadequate and coadministration is necessary, limit dosages and durations to the minimum required, monitor patients closely for respiratory depression and sedation, and consider prescribing naloxone for the emergency treatment of opioid overdose.
Tapentadol is a centrally-acting synthetic analgesic. Analgesic efficacy is due to mu-opioid agonist activity and the inhibition of norepinephrine reuptake. Mu-opioid agonism decreases ascending pain transmission to the thalamus, which is involved in conscious pain processing. Opoid agonists interrupt ascending pain signals in the spinal cord and enhance activation of the descending pain modulation pathway. Inhibition of norepinephrine reuptake enhances the descending inhibition of pain via activation of inhibitory alpha-2 receptors.
Tapentadol is administered orally. After systemic absorption, the drug is widely distributed throughout the body; there is insufficient information on excretion into breast milk. Minimum protein binding of approximately 20% occurs. Tapentadol is extensively metabolized to inactive compounds with only 3% excreted as unchanged drug after oral administration. Metabolism routes include conjugation via Phase 2 pathways via UGT (approximately 70%), methylation via CYP2C9 and CYP2C19 (approximately 13%), hydroxylation via CYP2D6 (approximately 2%), and unstated or unknown pathways (approximately 12%). Approximately 99% of tapentadol and metabolites are eliminated in the urine, with 1% fecal elimination, and no measurable respiratory elimination. The terminal half-life is approximately 4 hours with immediate-release tablets and approximately 5 hours with extended-release tablets.
Affected cytochrome P450 isoenzymes and drug transporters: CYP2C9, CYP2C19, CYP2D6, UGT
The major pathway of tapentadol metabolism is Phase 2 metabolism via UGT (70%), while minor pathways include metabolism via CYP2C9 and CYP2C19 (13%), and CYP2D6 (2%). Drug interactions with cytochrome P450 enzyme inducers or inhibitors are not expected. Tapentadol does not induce or inhibit cytochrome P450 enzymes.
-Route-Specific Pharmacokinetics
Oral Route
After absorption from the gastrointestinal tract, tapentadol undergoes extensive first pass metabolism resulting in an absolute bioavailability of approximately 32%. The drug does not require activation for analgesic effect.
Immediate-release preparations: Time to Cmax is 1.25 hours, and there are dose proportional increases in Cmax and AUC over the dosing range of 50 to 150 mg. The absorption of the immediate-release product is expected to increase when administered with food. In a human study, tapentadol administration with a high-fat, high-calorie meal increased the AUC by 25% and Cmax by 16%.
Extended-release preparations: Maximum serum concentrations are achieved 3 to 6 hours after administration. Dose proportional increases in AUC have been observed over the therapeutic dosing range. With twice-daily dosing, a steady-state drug concentration is attained after the third dose. Minimal accumulation has been noted with an oral dosing regimen of 250 mg twice daily. The AUC and Cmax increased by 6% and 17%, respectively, when administered after a high-fat, high-calorie breakfast. Administration with ethanol will increase drug bioavailability.
-Special Populations
Hepatic Impairment
Higher exposures and serum concentrations of tapentadol have been observed in patients with hepatic impairment. The ratio of tapentadol pharmacokinetic parameters for adults with mild hepatic impairment (Child-Pugh score 5 to 6) and moderate hepatic impairment (Child-Pugh score 7 to 9) as compared to adults with normal hepatic function were 1.7 and 4.2 for AUC, respectively; 1.4 and 2.5 for Cmax, respectively; and 1.2 and 1.4 for half-life, respectively. The rate of formation of tapentadol-O-glucuronide was lower in adults with decreasing liver function. Tapentadol has not been studied in adults with severe hepatic impairment or pediatric patients with hepatic impairment.
Renal Impairment
Tapentadol AUC and Cmax were comparable in adults with varying degrees of renal function (from normal to severely impaired). However, increasing exposure (AUC) to tapentadol-O-glucuronide, a metabolite with no analgesic activity, was observed with increasing degree of renal impairment. In adults with mild renal impairment (CrCl 50 to 79 mL/minute), moderate renal impairment (CrCl 30 to 49 mL/minute), and severe renal impairment (CrCl less than 30 mL/minute), the AUC of tapentadol-O-glucuronide was 1.5-, 2.5-, and 5.5-fold higher compared to adults with normal renal function, respectively. Tapentadol has not been studied in pediatric patients with renal impairment.
Pediatrics
Maximum tapentadol serum concentrations in pediatric patients 6 years and older were observed at times similar to adults, with no age related changes. Estimated simulated steady-state exposure (AUC ng x hour/mL [2.5th to 97.5th percentile]) was 309 (197 to 487) and 369 (233 to 563) in pediatric patients 6 to 11 years and 12 to 17 years, respectively, who received tapentadol oral solution 1.25 mg/kg/dose every 4 hours for 5 days, compared to 217 (131 to 353), 325 (196 to 529), and 434 (262 to 706) in adults who received 50 mg, 75 mg, and 100 mg, respectively.
Geriatric
In premarketing clinical trials, the mean tapentadol exposure was similar in elderly subjects compared to young adults with equivalent doses; mean Cmax was 16% lower in elderly as compared to younger patients.
